Statement: Dr. Umang Mehta is a double board-certified facial plastic and reconstructive surgeon. At Mehta Plastic Surgery in the San Francisco Bay Area, he specializes in rhinoplasty, which makes up about 95% of the procedures in his practice. “What sets my approach to nasal surgery apart is that I’m continually trying to improve my techniques and get better as a surgeon,” he says. “Whether you took a hard fall on a surfboard and broke your nasal bones or want to straighten out a hump and lift a droopy tip, I can make your nose much more functional and help you achieve your aesthetic desires.” He is an expert at both first-time and revision rhinoplasty. Plus, he has extensive experience placing incisions both outside (the open surgical approach) and inside (the closed approach) the nose.

Dr. Mehta’s plastic surgery philosophy is that patients should look natural and not like they’ve had work done. “For me, the goals are for the patient to look more refreshed and younger—to be a more refined and better version of themselves,” he says. To ensure he understands your aesthetic goals, Dr. Mehta uses computer software to “morph” your nose. The computer-generated images help you envision your nose after surgery and foster communication about your desires, so that you and Dr. Mehta are on the same page going into the procedure.

Dr. Mehta received a Bachelor of Science in Biological Sciences from Stanford University (graduating with honors) and his medical degree from the Ohio State University College of Medicine. He completed an internship in general surgery and a residency in otolaryngology at Saint Louis University School of Medicine. He then completed a fellowship in facial plastic and reconstructive surgery at the world-renowned Lasky Clinic in Beverly Hills, California, and the Keck School of Medicine of the University of Southern California.

A RealSelf Top Doctor honoree, Dr. Mehta is board certified by the American Board of Facial Plastic and Reconstructive Surgery and the American Board of Otolaryngology – Head and Neck Surgery. He is also a member of the American Academy of Facial Plastic and Reconstructive Surgery.

As a rhinoplasty expert, Dr. Mehta has lectured, taught, and published research articles and book chapters on the topic, including in the prestigious surgical textbook Master Techniques in Rhinoplasty. Dr. Mehta is also honored to be a Volunteer Clinical Professor at the University of California, San Francisco Department of Otolaryngology, Head and Neck Surgery, instructing residents and fellows in rhinoplasty and other facial plastic surgery procedures.

Rhinoplasty asymmetry due to scar tissue?
Asymmetry after rhinoplasty
Oct 21, 2023
Thanks for the question. It looks like your bridge and tip are both deviated to the right. This is not due to scar tissue but rather your nasal bones and the top of the septum being off center. Straightening these out can certainly be achieved in a revision rhinoplasty. There are ways to reduce the swelling more quickly (steroid tablets, steroid shots) if you were to undergo another nasal surgery. If desired, your tip could also be made more refined and symmetrical at the same time.

Sorry to hear you had a negative outcome from buccal fat removal. You can replace the lost volume with a long lasting collagen stimulator like Sculptra which can give your face a fuller and more youthful appearance. Be sure to have it done with an experienced injector to optimize your results.

Swollen 18 months after rhinoplasty. Is a revision inevitable?
Not too late for steroid injections
Sep 11, 2021
You should consider seeing a rhinoplasty specialist in your area. I’ve done a steroid injection in a patient who had a rhinoplasty 40 years ago (!) in Iran, and she noticed a nice improvement in her tip definition. She was able to avoid having a revision rhinoplasty. I do them routinely in patients well past the one year mark to reduce scar tissue and swelling. That would be a good first step followed by an assessment to see if you need a revision to reshape your tip in a more a...
Why did my nose got long again 1 year after rhinoplasty?
Nasal tip shape change after rhinoplasty
Apr 21, 2021
An in-person exam would be helpful but it’s certainly possible that you may have some scar tissue under the skin that could respond very well to a steroid injection. I use Kenalog plus 5-FU for this. If your surgeon is unresponsive, you could seek a 2nd opinion.
Kind regards,

Can diced cartilage grafts be taken out and smoothened? What options do I have for a revision?
Diced cartilage with fibrin glue for augmentation
Apr 4, 2021
Sorry to hear about your negative outcome from your rhinoplasties. It is absolutely possible to remove the cartilage fragments from your bridge and replace them with tiny diced cartilage pieces that are glued together with Tisseel. If you check out our Instagram feed, I recently posted this technique in a video and showed a case similar to yours. By dicing the cartilage finely and gluing it together, the bridge can be augmented in a smooth, natural, and predictable way. Rib, septal, and ear...
Do I need another revision rhinoplasty? Is there a procedure for thinning the skin & removing scar tiss
Revision rhinoplasty in thick skinned patients
Mar 20, 2021
Thanks for the question! I remove scar tissue and thin the skin envelope very frequently in both primary and revision rhinoplasty. This must be done very carefully to preserve the blood supply to the skin. I would have to examine you in person to let you know if you are a candidate... Sometimes if the cartilages were made too small inside the nose, there is empty space between the cartilage and the body fills this space with scar tissue. Often making a nose more defined in thicker skin...
Is it possible to add a small dorsal hump with nose filler?
Filler to raise the nasal bridge
Mar 2, 2021
Yes, nonsurgical rhinoplasty (filler in the nose) can be an excellent option for raising a bridge that was over-reduced during rhinoplasty. I use Restylane Lyft primarily for this indication. The filler is reversible if needed, and the results should last 1-2 years or more. In the future, surgical correction with diced cartilage could be an option, or you could do the filler periodically.

Dr. Mehta
Buccal fat removal or rhinoplasty first?
Cheek contouring and rhinoplasty
Feb 28, 2021
Thanks for the question! Based on these photos, I wouldn’t recommend buccal fat removal in your case because it would make you look hollow/gaunt. Rather, a filler for your upper cheeks would give your face more of a heart shape and less round/oval. You could start with something like Voluma, Lyft, or Radiesse and then transition to something longer lasting like Sculptra or fat transfer.

A rhinoplasty would certainly be reasonable too. Computer imaging can help you and your surgeon create a...

Would filler in my nose benefit me? I am planning on getting a real nose job next year. I am getting ch
Filler or surgery for your nose?
Jan 17, 2021
Great question! Honestly, looking at your nose, I think you'd be a much better surgical than nonsurgical candidate. A tiny bit of filler could be placed above your hump to smooth it, but I totally agree that your hump is so small that it's not bothersome. Filler placed at the base of the columella (the part of the nose between the nostrils) can rotate the tip, lifting it upward in cases in which the tip is droopy. But I think your tip angle Is fine as is.

With surgery, it would be possible...

Should I have a revision rhinoplasty? My nose looks so upturned
Revision rhinoplasty to lengthen an upturned nose
Dec 20, 2020
Thanks for the question. Your tip does look quite upturned at 4 months. While there still might be a little more "tip drop" happening over the next few months, it probably won't change too dramatically. That said, you do need to allow your nose to heal from your first surgery before seeking a revision. The usual rule of thumb is one year, though it might be possible to do the revision at 9-10 months if your swelling goes down quickly and there doesn't seem to be too much scar tissue....
Do PRP injections have the same risks as fillers when accidentally injected into a vessel? Looking to t
Volume restoration: fillers or PRP?
Dec 13, 2020
Great question! PRP is an excellent treatment for skin texture and acne scars when combined with microneedling or injected under the skin. It's not going to work for larger areas of volume restoration like the temples, cheeks, etc. that you had mentioned. Hyaluronic acid fillers are safe and effective when injected by experienced injectors. Radiesse is another commonly injected filler for larger volume areas. Another good alternative to these fillers is Sculptra, which is very long-lasting...
Would another revision be possible due to thin skin?
Revision rhinoplasty to correct a collapsed bridge
Nov 16, 2020
Thanks for the question and photos. I do agree with you that the lower part of your bridge is not as high as the upper part. The height of the lower part would be acceptable if not for the “bump” at the upper portion. You could either lower that bump or raise the rest of your bridge, depending on whether you want a scooped or straight look. I would have to examine your nose in person to assess whether your skin has adequate blood supply to safely undergo another surgery. In thinner ski...
Revision rhinoplasty: are my expectations realistic?
Rhinoplasty can be combined with revision septoplasty
Nov 9, 2020
Thanks for the question and photos. It is wise of you to plan to correct both the residual deviated septum and the outside shape of your nose at the same time. The cartilage from the inside of the nose is used to straighten, refine, and support the outside structure. Because you’ve already had a septoplasty, you might be missing some cartilage from your septum, in which case ear or rib cartilage may be needed. You should seek consultations with 2-3 surgeons who specialize in rhinoplasty a...
Does endoscopic septoplasty involve less side effects than non-endoscopic septoplasty?
Endoscopic vs regular septoplasty
Nov 7, 2020
Great question! An endoscopic septoplasty just means that the surgeon puts a scope (tiny camera) into the nose for visualization, rather than directly looking into the nose with a headlight and speculum. Endoscopes are very useful in endoscopic sinus surgery, to help the surgeon see the sinus openings deep into the nose. However, the septum is easily seen with a light/speculum, so I don’t find an endoscope necessary. Septoplasties are routine for most ENT surgeons, and most experienced o...

Kenalog injections after rhinoplasty
Nov 1, 2020
Great question. I use Kenalog injections often in my post-rhinoplasty patients. The injections are particularly crucial in patients with thick skin and those who have had prior rhinoplasties. The risk of skin indentation is low if performed correctly, and the indentations are generally subtle/temporary/reversible. The benefit outweighs the risk in my mind. Oral steroids can help too, but they don’t replace the benefit of Kenalog. I’d suggest seeking a second opinion, tbh. Pre- and postop Acc...
