While a full consultation with a board-certified plastic surgeon is needed, I do think that a formal neck/lower face lift, with or without repeat liposuction, is probably the best option to improve what I’m seeing in your pictures. I would stay away from fat grafting in this case and focus on a subglandular breast augmentation with “radial scoring” of the breast tissue; I would probably also recommend an areolar reduction.
