It's more than likely that the injury caused some minor weakening of your lower lateral nasal cartilage and buckling, which could explain the indentation you ended up with. These types of injuries typically to not reverse themselves, and if you are experiencing pain and bleeding at the site you may have an infection/folliculitis around a nostril hair. This may require further medical attention and I would recommend and evaluation by an ENT, facial plastic surgeon or plastic surgeon

4 weeks is incredibly early after a rhinoplasty. Based on your photos there is a considerable amount of swelling and this can alter structures that may have been left alone.
It is incredibly difficult to be patient after a rhinoplasty for the final results, but it can oftentimes take up to a year for the nose to settle down. Make sure to continue with icing and regular follow up visits with your surgeon.

I'm sorry that you are unhappy with your result from your surgery. Healing from any procedure is an emotional experience and can be hard to navigate.
It's difficult to give someone back their "old nose", exactly the way it was before as cartilage has been reshaped and in some cases removed entirely. A columellar strut graft can come down a bit more as it is not secured to your septal cartilage. You do have thicker nasal skin which holds onto swelling longer than someone with thinner skin. You're still fairly early in the post op period and I would recommend giving your nose the time to heal. Remember, it takes up to a year sometimes even a year a half to completely heal from a rhinoplasty.

5-6 months out from a revision is still fairly early and the fullness in the nasal tip may continue to come down. The asymmetry of your nostrils is likely due to a mismatch in cartilage at the tip of your nose. The hanging tip that you see may come up slightly as you are still recovering from your most recent revision. Before rushing into a third procedure, I would recommend waiting a full year to ensure no further changes to your nose occur.

The changes to your nose are visible, but a revision rhinoplasty will not guarantee a return of your original nose. From your pictures I do not see a mid-columellar incision, but these frequently heal so well they are almost invisible post operatively. You could have cartilage grafts placed in the desired area, but I would say rib would not be necessary for these changes. Your are very early in the post op period, and if you are one week out, very likely your nasal splint was just removed. Everyone always swells immediately after the splint is taken off and the swelling can increase if you attempt to do too much physically or are out in the sun excessively. If you feel bone where the swelling is, that is likely the site of your lateral osteotomies or where the nasal bones were broken and this can take months to calm down.

The are around the tip of the nose can hold on to post operative swelling for a frustratingly long time. While the initial healing and majority of swelling after a rhinoplasty calms down at a reasonable pace, the full results can take from one to one and a half years depending on amount of surgery, skin thickness, and if it was a revision or not. The key is to be patient with the healing process! You've made it through the hard part already!

Changing the tip, or rotation, of your nose depends on the final look that you are going for. Currently your nasal tip is in a good position. Making your nose look shorter or longer with a dorsal hump reduction would depend on the amount taken off and if anything is done to the tip at the time of surgery. I would recommend that you go through your photos with your surgeon using morphing software so that he or she can be more clear in the anticipated results.

Two weeks post operatively is very early in the healing process and asymmetry that is apparent early on can subside as the swelling calms down. The swelling by your eyes could be swelling if lateral osteotomies were performed. The dip that you see looks like uneven swelling. It's possible there was a graft placed along your nasal dorsum, and this could be a reason for the asymmetric swelling. Hello, I'm so sorry to hear about your experience. It does look like there has been tissue loss. You could consider a small wedge excision of the area where there has been tissue loss or try a temporary filler to see if this addresses the issue as well. There is a small chance that further surgery may results in a similar result, so make sure you see a board certified facial plastic surgeon or plastic surgeon who has surgical experience with the lip.
