This is a 73-year-old woman who had saline implants placed on top of the muscle 40 years ago. She came to me wanting improvement in the appearance of her large, sagging breasts. She wanted to be smaller, but also wanted to regain the full appearance her breasts had when they were first augmented. To accomplish this I perform an operation called "Reductive Augmentation," in which a small silicone implant is placed under the muscle, while bulky tissue from the bottom of the breast is removed. In her case, her 175 cc saline implants were removed, 265 cc high-profile silicone implants were placed under the muscle, and about 300 grams of breast tissue were removed from each side. The result is lifted breasts, smaller in size, with good upper pole fullness and cleavage.
