Thank you for your question.The time required for a bilateral DIEP free flap during a prophylactic mastectomy can vary based the time required for the mastectomy and for the reconstruction.  In experienced hands, barring any complications, this operation can be done within a 5 hour window.  Longer surgical times are associated with increased recover time and slower return to daily activities.  Good luck with your surgery

Thanks for your question.  With a DIEP flap an insignificant amount of fascia MAYBE removed and the closure of the fascial (Anterior Rectus Sheath) will cause minimal tightening of the lower abdomen. Usually during a second surgery to correct of scar position of the DIEP donor site, any bulges in the abdomen are addressed and a full plication will be performed in possible to restore the abdominal wall integrity and core strength.Hope this helps.

Thank you for your question. At this time the top implant manufacturers have met the FDA guidelines for breast augmentation and reconstruction.  In my practice I use Allergan and occasionally Mentor implants.  Smooth round implants work the best if place properly.Hope this helps.
Can I remove fibroadenomas and have reconstruction with implants at the same time?

Thanks for your question. Fibroadenomas are generally benign lumps in the breast that are removed with having to do a mastectomy.  If the breasts are deformed after removing the fibroadenomas then breast reconstruction can be utilize to restore the breasts.

Thanks for sharing your picture.  Having had several abdominal surgeries puts you at risk of having intra-abdominal adhesions and also perhaps a periumbilical hernia that could manifest with the pain you are experiencing.  After some weight loss, you would benefit from liposuction with a tummy tuck.  If there is a hernia, that can be addressed during your tummy tuck.  Good luck and I hope this helps.

Yes these two treamtens can be performed on both areas at the same setting.  We typically treat the abdomen first and then buttocks.  Since its an hour of therapy on the abodmen and the buttock together, make sure you are hydrated and have a healthy meal after.

Thanks for your question. Dietary changes ands stress the way your body looks and how you feel. Its possible to get back into shape if you have now changed back to a healthy diet.  I would consult with your board certified plastic surgeon and come up with a game plan that works for you.

Thanks for your question. It’s generally considered safe to expand these tissue expanders to some extent over their stated fill volume. However it’s impossible to expand them to twice their volume. Other options are available if you’d like to have larger implants. Consult with your plastic surgeon.
