This is not a sign of a medical problem or of a disease. If you don't like the appearance of the protruding labia minora, then you might be interested in a labiaplasty. If not, you can leave your labia alone.

Your labiaplasty should have included excision of this loose skin. Since it wasn't done, it can be done now. It needs to be done with precision to avoid pain issues afterwards. There is no unusual connective tissue in this area and there is nothing risky about it in experienced hands. It shouldn't require a perineoplasty, but that's an option to consider if the vaginal opening feels loose during sex.

All surgeries have risks. The only way to damage the dorsal nerves (there are 2 of them, not one) is to cut them or burn them. In theory, every surgeon who does a labiaplasty should know where these nerves are and use that knowledge to avoid cutting them or burning them. In reality, some surgeons don't know where these nerves are, don't know that they even exist, and that's why there is still a risk.

No. It is not okay to get these treatments. First, these procedures will break your hymen. Second, they will not tighten your vagina. Third, when you say that you feel that your vagina isn't as tight as it should be, I don't see how you can make that determination. Don't let anyone trick you into getting these procedures done. Tissue that has been cooked recently with laser doesn't heal as well from labiaplasty as tissue that hasn't been recently cooked. For that reason, it would make sense to defer any laser therapy in that area until healing is complete. At least 6 weeks. Preferably 12 weeks. However, UltraFemme is not a laser treatment. It is a radiofrequency treatment and doesn't generate as much heat as a laser, but the same recommendation would still apply.

If one "top" female genital plastic surgeon can't do a simple excision of an anal skin tag, go see another one or a cosmetic gynecologist. It's a quick procedure. I do most of these under local and usually in combination with other aesthetic procedures in the area.
