If I have a nonsurgical rhinoplasty first, will it jeopardize a positive outcome or esthetics of
Nonsurgical rhinoplasty before a surgical rhinoplasty
Sep 17, 2019
Thanks for your question! Haivng a nonsurgical rhinoplasty before a surgical rhinoplasty can create problems during the surgical procedure, but those complications are rare. In a patient that has had filler to the nose before, the primary concern when undergoing a rhinoplasty is compromise of the skin envelope if the filler was placed too superficially (close to the skin). Sometimes additional soft tissue grafting is required if the skin was damaged or is too thin.

If rib cartilage warps, then how can it possible be used as spreader grafts?
Rib cartilage warping
Jul 30, 2019
Rib cartilage warps to varying degrees primarily based on calcification of the rib which increases as you get older. In younger patients there are very few calcifications so the rib warps a lot. In contrast, older patients have more calcifications so it warps much less. Irradiated rib still has some warping tendencies as well but usually is pretty minimal.
Rib cartilage can very safely be used for spreader grafts, you just have to harvest the rib early, carve it into the…
What could cause fluctuating swelling 8 years post rhinoplasty?
swelling after rhinoplasty
Jun 19, 2019
Unfortunately this is something that can be seen after rhinoplasty especially in people with thick skin. Now assuming that you don’t have an implant like goretex or silicone or something else in your nose, then it may be simply be related to the face that you’ve undergone a rhinoplasty. While not common, after surgery sometimes you can see daily or weekly fluctuations in swelling. Most of the time patients will complain that exercise, excess heat, alcohol (especially wine)…
Do I have a droopy tip or an illusion of droopy tip? What can be done?
Drooping tip
Jun 19, 2019
To answer your question directly, technically the tip is not drooping. What you’re seeing when you smile is an “over-projected caudal septum” where there is excess length in the septum. This is also what creates a slight crease in the upper lip when you smile. That combined with the activity of the depressor septi nasi muscle creates the illusion of a drooping tip when smiling. You can also see a little bit of hanging columella where the distance between the…

Cartilage grafting in rhinoplasty
Jun 19, 2019
Cartilage grafting in rhinoplasty is incredibly safe and works very well as long as the cartilage is used properly and placed in the right areas. It is incredibly rare, but possible, for cartilage to resorb or “go away”. Typically this is only seen if there is an infection present. It’s also debate-able on whether or not donor or cadaveric cartilage resorbs over time but many people use it safely with good long term results.
As with any rhinoplasty, its all…

Under eye treatment
Jun 19, 2019
It’s difficult to make a full recommendation based on the photos, but it appears that you may have some volume deficiency of the lid-cheek junction which can be improved with both filler and fat transfer. But I would definitely tailor the recommendation based on an in-person exam of your eyes and the area around the eyes. Make sure you see a facial plastic or oculoplastic surgeon, best if they are comfortable with fat transfer. Good luck!
What is the risk of a lip palsy during a minimally invasive face lift and how could it be correct
Lower lip palsy during facelift surgery
Jun 19, 2019
The risk of developing a lower lip weakness during facelift surgery is variable both based on surgeon and technique. “Minimally invasive” techniques typically do not have very extensive dissection and the risk is incredibly low. More extensive dissection and surgery puts the nerve at higher risk but in experienced hands the risk is very low. If a nerve injury occurs, the single best option is to repair the nerve at the time of injury. Other procedures can be done in a…

Recovery after rhinoplasty
Jun 18, 2019
You’re kind of in the “too early to tell” stage where you’re a little soon out from surgery to be able to tell you what might be happening. It could be a wide variety of problems including asymmetries due to swelling. In reality your surgeon would be best able to answer this question but if you’re not comfortable with them then you could always seek another evaluation but without having performed the surgery, most of us would tell you to wait and see how the swelling settles…
What is the best treatment to get rid of these under eye wrinkles?
excess lower eyelid skin
Jun 18, 2019
Excess lower eyelid skin is typically best treated with blepharoplasty to remove the excess skin. However there are various options to create a “tightening effect” but it really depends on how severe the problem is which can only really be determined by an in person consultation.
Lower (and upper) blepharoplasty is commonly performed in conjunction with face/necklifts as well as with other adjunctive procedures like peels, fat transfer, etc. An in person evaluation would be…
What can I do about my neck and jawline as I don’t recognize my side profile anymore?
Side profile
Jun 18, 2019
Congratulations on your new baby! Pregnancy and the post-partum period can change the body dramatically and you may be seeing some of those effects.
I would recommend taking time to recover as weight and hormones change dramatically after pregnancy and until you’ve stabilized I wouldn’t do anything aggressive. Things can change quite a bit and that fullness underneath the chin may subside slightly, which may improve your side profile back to where you are comfortable with…
What can I do now as I think I’ve broken my nasal bones?
Nasal bone fractures
Jun 18, 2019
Breaking your nose is never convenient, especially when you’re on vacation. The best thing you can do is try and get evaluated locally to make sure nothing needs to be done in a more urgent time frame. Nasal bone fractures can be “reduced” or reset within the first couple weeks after a fracture if they need to be so its best to seek evaluation quickly if possible.
Hopefully, if a break is truly present, then you may just be seeing swelling at the fracture site and callous…
Do I need alar base reduction, sill reduction or both?
Alar base reductions
May 7, 2019
It really depends on a full facial and rhinoplasty analysis as well as exactly what you’re looking for to make the decision on alar base reduction techniques.
Personally I never make the final decision on alar base reduction until the end of an operation because maneuvers during rhinoplasty can dramatically alter alar base insertion and nostril flare. For example, if part of your rhinoplasty plan is to increase tip projection, then this will usually narrow the base width…

Asymmetry after neck lift
May 6, 2019
It’s hard to tell you whether it’s normal really without examining you and feeling the skin and underlying soft tissue. It is reasonable to try a simple injection in the office as it is low risk and could improve. In the event that there is some asymmetric swelling you may see some improvement but time will tell.
Continue to followup with your surgeon and express your concerns going forward! Good luck with your continued recovery!

Facial fat grafting and facelift surgery
Apr 29, 2019
Personally I would do both of these simultaneously. Restoration of volume at the same time as removal of excess skin and re-supporting the deeper layers of the face is a powerful procedure that can give an excellent aesthetic result.
If you have a surgeon in mind that you want to go see, I would go ahead and schedule a consultation and discuss these questions directly with them! Good luck moving forward!

Rhinoplasty result
Apr 29, 2019
Thanks for your question! You’re definitely in the “too early to answer” stage. Rhinoplasty recovery takes a long time and you can continue to see changes up to and beyond a year. It’s hard to say exactly based on your photos but if you have thick skin, then the recovery can take even longer.
At this point I would try your best to remain optimistic and let the final result show itself to you in time. If you’re still unhappy with the result then an in…
