Otoplasty - Reducing the size of the concha for macrotia. Suggestions?

Hi there! Reducing the length of the concha is difficult as it will not really maintain any normal shape if you try to pinch it together. We can reduce the "width" easily when combined with a fold. Would need to see the front view to see how much the concha is visible in that dimension. Typically reducing the earlobe is the simplest fix but it doesn't appear to be prominent in your case. Typically we want the ear to be 60-65mm so you are slightly elongated.
Should I have incisionless otoplasty or the more traditional otoplasty with an incision?

The left ear would be incisionless and the right ear traditional. Secondary to that I'd go with traditional with incision! You need a consultation to explain the details of why! Don't let any surgeon tell you that you MUST go to sleep. That is likely because they don't do the surgery that often. Also ask to see all the before and afters. They should have 30+ if they are truly experienced in the surgery!
Buccal Fat Pad damage. Would I need to remove them to fix the problem?

There is a problem called psuedoherniation of the buccal fat pad. If you do have this then removing the fat will be the answer. Find a doctor who does A LOT of buccal fat removal and is experienced, get the diagnosis, and then get treated!

I think it would provide a nice subtle result for you. I do not think it would over hollow your face nor would it create a GIANT change but it would be similar to "losing weight."

First off, I agree with the elder statemen's opinions below that you are not a candidate for buccal fat removal based upon the shape and contour of your face.
That being said, for others reading this, buccal fat removal can ABOSULTELY be performed in older individuals. It is often done in conjunction with a facelift which is why it is shocking to me to hear so many other surgeon's say that it will make you look aged.

I do not think you need general. I have had a patient with a severe gag reflex where I had wished it was under general. You should take something like Valium to relax you as it lowers your blood pressure and that makes it so that you have less bleeding/swelling.

I think buccal fat removal would certainly give you a more sculpted look. In general you want to tighten the jawline (lipo helps here); decrease the midface roundness and increase the cheek bones (implants, fat grafting or filler). Hope that helps!

I do think both would help however generally find that Botox to the masseters is not remotely a cost effective solution. Considering you can do an entire buccal fat removal for $2500; spending $400 every 3 months for masseters botox is not a great value.

I often measure the amount of fat taken out as I take the fat and reinject it into the face. When I do this I would say the average patient gets 3cc. Most surgeons will take out what is given to them for most patients. Your baseline weight, age, gender, ethnicity and genetics all play a role on how much you have.

It is unlikely that the excision was uneven. Even when uneven amounts are removed it usually is a small difference. More likely than not you have some baseline bony asymmetry that has become more obvious now that you are actually looking at your face in more detail. Regardless, you CAN undergo a revision buccal fat removal; however there is no guarantee the fat will be in there and easy to remove.

It is unlikely that is buccal fat. Sometimes you can get "psuedo-herniation" of the buccal fat and that is a dynamic fat that comes and goes. That to me looks like perioral fat which can be removal with gentle micro-liposuction.

WIthout pictures this is difficult to assess. For your age a facelift is out of the question. I would consider a buccal fat removal with grafting to the malar fat pad to augmental the cheeks but this is impossible to say without seeing pictures at the very least.

It depends on what you are looking to invest and what your goal change is. I'm sure you would agree that no one would look at your face and say it NEEDS to be more defined and chiseled. If you are looking for a drastic change you would need bony augmentation with implants (mandible and cheeks). That being said a slight improvement with minimal downtime, minimal cost and easy experience might be a quick buccal fat removal. Good luck to you!
