I am 40 year old male with fat pads under my eye. How would I go about getting malar fat pad remo
Eye bags
Jun 18, 2020
Hi alexm1980-Great question. This is a common finding in young patients such as yourself and one of the earliest signs of aging. There are a few options. A minimally invasive option would be placement of a filler into the “tear trough” to help camouflage the bag. The other option would be a surgery called a blepharoplasty. In your case, the blepharoplasty could be done through the inside of the lid to remove and/or reposition the fat. I would recommend consultation with a surgeon to discuss …

Scar
Jun 5, 2020
This is likely a deposit of scar tissue. I agree with massage. If it does not resolve on its own a small steroid injection can be helpful. I recommend you see the surgeon who performed the surgery.
Would you recommend getting a nose piercing before or after a non surgical nose job?
Nose piercing
May 12, 2020
Hi beauty305_
I would discuss with the surgeon you chose to provide the treatment. It depends on where the piercing is and where the filler is to be placed. You do not want to introduce bacteria into the filler as it can form a “biofilm” and a difficult problem. Hope this helps
Would I benefit from a lip lift?
Lip lift
May 12, 2020
Hi azgirl2123
A lip lift is a great option for increasing the red lip and dental show, while decreasing the height of the white lip. It is a relatively simple procedure but requires meticulous attention to detail from the surgeon. There is a scar just under the nasal sill/base, but it should not be noticeable in most situations. The one exception is if light is coming from below your chin, such as while sitting at a candlelight meal- in these situations the scar may be more noticeable…
What are these brown rings around my eyes? How can I treat them?
tear trough
May 12, 2020
Hi improvingmexo-
This is a very common concern, even in young patients like yourself. The hallowing below the eye is called a “tear trough” and forms as we get a little older. The shadow caused by the depression causes the dark circle to appear. A soft hyaluronic acid filler is a great option to correct this. Sometimes there are also pigment differences in the lower lid skin.
What caused my face to flatten and lose my cheekbones?
Facial volume
May 12, 2020
Hi Thrifty 4911-
Great question! It is a little hard to see from the photos what changes might have occurred. Laying down, sleeping etc can cause temporary changes in the appearance of your face but will not cause permanent changes. However, our face does change as we are or gain/lose weight. For augmentation of the cheeks and jawline/chin, fillers are a great place to start. There are more permanent options but I would recommend starting with fillers. I recommend consulting with a plastic…
What are my options to narrow and define my nose tip? Are there less invasive treatments? (photos
Revision rhinoplasty
May 4, 2020
Hi mt2480-
I agree with your assessment. While it is attempting to consider fillers in this situation I would be cautious about using fillers in the tip of the nose after a rhinoplasty. If you want to consider this option I would discuss with a surgeon who performs surgical and non-surgical rhinoplasty. As for a revision, you may be able to get an endonasal (closed) revision with placement of a small tip graft for better definition through your thicker skin. Good luck!
How to get rid of folds/dents/lines near lips and chin?
botox
Apr 17, 2020
Hi SNaveed786-
Great question! A full examination would be helpful to address the areas of concern. One common complaint is that of dimpling of the skin in the chin. This is typically due to over activity of the mentalis muscle and can be addressed with botox to that muscle. Hope this helps!
Are There Plastic Surgeons Who Work with Adults with Cleft Lip?
Cleft rhinoplasty
Apr 17, 2020
Cleft rhinoplasty is a challenging operation. I often will revise the lip at the same time as the scar can be advanced to make the nostrils more symmetric. I recommend seeing a surgeon who both performs a high volume of rhinoplasty and cleft surgeries.

Bilateral cleft lip
Apr 17, 2020
Hi-
I would recommend seeing a surgeon who performs cleft lip and palate surgeries as a full examination would be helpful. Minor revisions can typically be done to address the lip if needed. Of course, I always recommend to my patients to cease smoking! Good luck.
What is the best way to make my face rounder?
Facial Volume
Apr 17, 2020
Hi Catsadles-
As we age we all lose volume in our face. This process is accelerated in some and can be affected by behavior to some degree. Most often, I will see avid runners have an exaggerated early loss of facial volume which is reflective of overall low body fat. Sometimes regaining weight will help, but we often see that facial volume changes independent of overall body weight. I do think fillers can help, and sculptra is a longer lasting treatment, but does require multiple…
What can I do about my teeth that don’t show when I smile or talk?
Lip Lift
Apr 2, 2020
Hi shreja
A lip lift is a great option for you. Be sure to see a physician with experience with lip lift as the procedure is relatively simple but there are some key steps to help minimize the visibility of the scar. Good luck!
What are the risks of rhinoplasty/septoplasty with rib and is ear grafting better?
Rib cartilage
Apr 2, 2020
Rib and ear cartilage are different and not always interchangeable for grafting in the nose. I harvest rib frequently, and sometimes at the same time take ear cartilage for a different purpose, and patients often tell me the ear was more painful in the healing process! There are no long term risks from rib cartilage harvest, and the significant short term complication (pneumothorax) is thankfully very rare. Cadaver irradiated rib is an option as well and I will use it in older patients or…
Can my nose skin thickness be reduced?
Rhinoplasty
Apr 2, 2020
Hi samope-
There are some options to help with thick skin but most topical and light treatments are modest in their benefit. I suspect a rhinoplasty would be helpful in your case. There are specific techniques for addressing thick skinned individuals to project and refine the tip of the nose. Not infrequently, a reduction in the size of nostrils can be beneficial as well. I hope this helps and good luck!
I’d like to avoid invasive surgeries in general if possible? Would a filler help?
Pros and Cons
Apr 2, 2020
Hi sdebbie93-
I would recommend you see a surgeon who offers both non-surgical rhinoplasty and surgical options. An examination is key. Based on your photos, I think you would have a very modest amount of improvement with filler alone and if you were my patient I would suggest that you consider surgical options before investing in filler that may leave you underwhelmed. Of course, the major advantage of filler is that it is temporary and reversible, so if you weren’t satisfied, surgery…
