A dedicated innovator and scholar, Dr. Weiner was an early adopter of Botox for treating spasms of the face and neck as well as TMJ disorders. As you alluded to, these can all be performed on the same day. Therefore, there is no timing issues. I can do these procedures at one visit. If Restylane is being used as a "spacer", I'm not sure that there is much benefit. If Restylane is being used as a filler for atrophic scarring, there is definitely some benefits.

I would treat with 1064 laser, either Laser Genesis or Aerolase. It will take a few treatments. You also need to be on retin A and hydroquinone. Also consider chemical peels prior to the laser treatment.

If the area is non tender and is not getting progressively worse, it can be treated with a vascular laser. There are a small percentage of patients who have redness related to the Radiesse. 81% experience redness in the first 12 months after injection, with 4% noting to have severe redness per the package insert.

I should take 1-2 syringes of Radiesse for the forehead. It should be done deeply, along the periosteum, with cannulas. This is a high risk area and much be performed by a very experienced injector. Agree that it is better than fat for this area.

You can begin saunas about 24 hours after the procedure. The only negative impact is increased bruising and if thats not an issue or concern, you can do it sooner.

There should be no interaction of the previously placed Radiesse with fat grafting. Fat grafting is a good choice for temples but has to be done by an experienced surgeon. Hyaluronic acid fillers give the best results. I use Refyne in the dermis and add subcision for best results. There is a collagen response to this technique which should give a result between 1-2 years, but everyone is different. I have started to use RHA 2 as well, another HA filler for this too.

I would treat this with TCA Cross. Possibly add the microneedling as well. eMatrix can't physically get into this area and I wouldn't do it.

There is absolutely no effect on fillers with the Excel V laser. There are several peer reviewed papers that support this. Fillers are sterilized during manufacturing to temperatures much hotter than the Excel V laser for 10-20 minutes.

Welting is possible after Excel V laser. Topical, oral, or IM steroids can be used to reduce the swelling and improve the healing time. It doesn't appear to be a burn which would look like severe redness and possible blistering. Sleeping upright on 2-3 pillows for 2-3 days will reduce the swelling faster as well.

There is no problem with undergoing laser procedures when breast feeding. There is no affect on your breast milk. The Excel V laser is an excellent choice for diffuse redness and telangiectasias.

There is absolutely no risk to your fillers or Botox with the Excel V laser. There are several peer reviewed papers supporting this. The HA fillers from Restylane are heated to 120 C for 15 minutes to sterilize them. The heat from the laser is less than 100 C and only milliseconds. Also, Botox is absorbed by the nerve endings within 24 hours so a week is plenty of time to wait after that treatment.

The Excel V is a laser which has 2 wavelengths, 1064nm and 532nm. A laser is collimated of a single wavelength. IPL is Intense Pulsed Light and consists of broadband of light (multiple wavelengths) which is narrowed to small spectrum using filters. In general, lasers have more energy and are better to target reds and telangiectasias. The only advantage of the IPL might be in diffuse redness such as rosacea but that's debatable. This is not a branding issue, its 2 different technologies....

The Excel V is perfect for this area. Sometime, the lower lid can be pulled below the orbital rim and the laser is safe to use with metal eye shields. If the lid cannot be pulled far enough down, cornea shields must be insert prior to treating this area. Don't worry about the meibomian glands because this only requires one or 2 pulses of the laser.
