It is often best to address the whole nose at the same time, however once could solely focus on tip changes. A bolbous tip can impose limitations on the degree of definition and improvements one can expect from a rhinoplasty but still significant improvement of those factors can be accomplished in the right hands.

It generally prefer to dissolve the filler before rhinoplasty if possible, but in cases like this that some residual filler might still be there the surgeon can still visualize the filler and remove it during the surgery (or at least for the most part).

Empty nose syndrome is due to aggressive turbinate reduction. This can be done at the same time as any type of nasal surgery. There are many ways to reduce the size of the inferior turbinates and more conservative techniques can minimize the chance of such a potential problem in future.

It is very difficult to recommend filler options without having photos. Cheek is one of the most common areas to be treated with fillers and both augmentation and also improvement of asymmetries can be achieved with proper filler technique.

Yes of course it can be revised. A turned up tip is one of the common reasons a lot of primary rhinoplasty patients are not happy and with proper cartilage grafting technique (as you can see some examples of which in our photo gallery) the rotation and projection of your tip can be adjusted in a more optimal position while addressing asymmetries of your septum and nostrils.
Can I reduce Nostril size in my profile view with alar reduction?

Unfortunately alar reduction cannot or should not be evaluated like this and particularly on a profile view. That is a maneuver that often gets assessed with a full rhinoplasty assessment, Even though it can be done as an isolated maneuver, that will be usually following a rhinoplasty that has already addressed other more fundamental tip and alar issues.
If I place a filler on top of my chin implant will it develop a biofilm or an infection?

 That is a very good question. Fillers are considered biological implants so even a superficial filler can technically increase the risk of infection if it is close enough to the original implant. The risk is overall low however it’s not zero.

This is a complicated issue. There are multiple options to address and notched ala. Another composite graft can be attempted but also an alar rim graft or alar repositioning technique might be appropriate options too depending on the individual factors and exam findings.
Can I have a rhinoplasty after I had Botox for bunny lines?

Rhinoplasty can always be done after btox treatment of the bunny lines. As far as fillers, we prefer to dissolve the filler first and space the surgery at least 1-2 weeks after for optimal delineation of the nasal tissue and profile.

Proper cartilage grafting of the tip and nostril area is key to prevent pinching problems which are considered an aesthetic problem and also a functional one by constricting the airway. I'm not sure when you had the original repair but, to be on the safe side, I would wait for at least 3 months to undergo a second repair.
