Best facial filler enhancements for hollow tear troughs and fine lines?
Treatment suggestions
Mar 14, 2022
Thanks for your questions and helpful photos. It would be best for you to see a board certified dermatologist or plastic surgeon in person for an evaluation of how your face moves, check the skin and structures of the face under different lighting, and to talk about your goals. Fillers are usually a good option to start with, along with some preventative neurotoxins such as Botox, Xeomin, or Jeuveau. Make sure you are using a quality mineral sunscreen every day to protect your investment…
4 years after under eye dermal filler. Problems have remained – what can I do?
Undereye problems
Mar 14, 2022
I would definitely recommend seeing an oculoplastic surgeon for treatment of this area. A lot can go wrong with filler placed directly under the eyes, and only a very skilled expert should do any further work on that area. You will need to have whatever filler is remaining dissolved with hyaluronidase if you had a HA filler placed originally. Make sure you know what product was injected so your surgeon can come up with a management plan. Fillers that are not hyaluronic acid, such as…
Should nasolabial fillers be done at the same time as filler for acne scars?
Different fillers at the same appointment
Mar 9, 2022
Thanks for your question. Definitely multiple types of fillers can be injected in one appointment, but your dermatologist might want to do one area at a time to see how the first treatment settles. Sometimes you might not need as much as originally planned if you get a lot of improvement from one treatment. Talk to your doctor about the plan and I am sure they can give you an explanation for why or why not they might treat several areas at once. Please see an expert, and best of luck!
Can Spironolactone help prevent post-pill acne?
Spironolactone for acne
Feb 21, 2022
Thanks for your question. I would definitely ask your dermatologist about options. I would think that stopping the birth control and seeing if the spironolactone is keeping the acne at bay enough for you would be a very good option. Also having a topical regimen will be helpful. Please see an expert and best of luck!
Are there long-term effects to using fillers/partial buccal fat was removed?
Options for facial volume
Feb 21, 2022
Thanks for your question. I am not sure where you read those long term effects of fillers – that is simply not true. Maybe the author meant that use of neuromodulators, such as Botox and Xeomin, can cause some muscle atrophy, but I have not heard of fillers doing that. Anyway, I would recommend a consult with a board certified plastic surgeon or dermatologist with experience in revolumizing the face. They will be able to assess your face at rest and in motion and help come up with a…
How deep are DAO injections?
DAO treatment
Feb 18, 2022
Thanks for your question. Treating the DAO is a very simple and quick procedure with great results when done properly. The needles are very tiny and the medication is injected quite superficially in the muscles of facial expression, so not deep at all. Please see an experienced board certified dermatologist or plastic surgeon with experience in treating this area, and I hope you have a lovely outcome!
Dimple on top of forehead – can it be treated with Botox?
Botox for dimple
Feb 18, 2022
Thanks for your question. The answer depends on whether the area in question is tethered to the underlying tissue or just skin that is dimpled in . You could always try Botox to see if it helps and the good thing is, it wears off after a few months, so you are not making a long term commitment if you don’t think it helped. The forehead can take anywhere from 8 units to soften, up to 20 in some pateints with very strong frontalis muscles and a high forehead. Ask your dermatologist about…
Injecting stronger Botox solution. Is this safe?
Botox dilution
Feb 18, 2022
Thanks for your interesting question. Any provider can dilute the toxins to a concentration they are comfortable using. Some studies in the dermatology literature are showing that higher doses of total toxin have a longer lasting duration. You can also try Xeomin or Jeuveau which are slightly different but essentially have the same efficacy and duration as Botox. Ask your provider for options and if they are well-trained, board certified in dermatology or plastic surgery, and have a good…
Has my filler migrated or is it just in a poor location?
Placement of filler
Jan 14, 2022
Thanks for your question and helpful photos. I honestly think you look great and I would not dissolve the filler unless it caused you a lot of distress. It will eventually be metabolized, and in my opinion it does not look obviously migrated. Take care!
Going on 1st round of Accutane – will this help the comedonal acne, or only the cystic?
Accutane and types of acne
Jan 3, 2022
Thanks for your question. Accutane is effective for pretty much every type of acne. Depo Provera will not “counteract” with the Accutane, but might make your acne more stubborn to treatment. You might want to explore different contraceptive options with your dermatologist. Or, you can see a board certified OB/Gyn physician that can discuss long term contraceptive options with you. One medication to consider is spironolactone, which helps in many cases of hormonal acne. Please see an…
Which would be better: Long term microdose or another full round of accutane?
Long term Accutane use
Jan 3, 2022
Thanks for your question and helpful photo. You might very well be a good candidate for long term, low dose accutane. Please discuss this with your dermatologist. If you have also taken spironolactone and had not had great success with that, accutane might be your best bet. Please see and expert, and best of luck!
How effective is Botox for hyperhidrosis of the scalp? Are there potential side effects (such as
Botox for hyperhidrosis
Jan 3, 2022
Thanks for your question. If you feel that hyperhidrosis is a significant problem, definitely ask your dermatologist about Botox treatments. It is unlikely that it will cause any thinning of the hair or hair loss. Botox is a temporary treatment, and will need to be repeated a few times a year, but might be helpful for you as the benefits probably outweigh the risks. Please see an expert, and best of luck!
Won’t use Lidocaine for Tattoo Removal because staff said I need to feel to ensure laser is worki
Use of lidocaine injections for tattoo removal
Jan 1, 2022
Thanks for your interesting question. I agree with “everywhere else” in that you don’t need to feel pain to have an effective removal session. We board certified dermatologists who are properly trained in laser physics and also the use and risks of lidocaine are comfortable injecting fairly small tattoos ( you don’t want to give some lidocaine toxicity!) and you just know how to watch the tissue reaction to determine what settings to use. Also the manufacturers of legitimate, FDA…
Is 3 months an optimal time to wait between laser tattoo removal sessions?
Timing of tattoo removal sessions
Jan 1, 2022
Thanks for your question. I would imagine a lot of people have the same type of question. The answer really depends on a lot of factors. Older tattoos – ones older than 5 years- have a better track record of being easier to remove. Also the type and color of ink matters. It also depends on what type of laser your dermatologist is using. Traditional Q-switched Nd:Yag lasers work great, but some people feel the pico-second Nd: Yag can be faster. The pico lasers cost a lot more for your…
Do tattoos fade more quickly the more sessions you have?
Timing of tattoo fading
Jan 1, 2022
Thanks for your question! I am glad your tattoo is fading to your satisfaction. You are probably right in your sense that the tattoo seems to be fading more quickly the more treatments you get. In the beginning of your treatment plan, you probably had very dense, concentrated ink under the skin, and if the laser treatments are done properly with the right type of laser, the ink literally explodes microscopically under the skin, and your immune cells called “macrophages” take care of the…
