Statement: Dr. Joseph Michaels is a board-certified plastic surgeon serving patients in the greater Washington, D.C. metropolitan area. At his practice, Michaels Aesthetic & Reconstructive Surgery, Dr. Michaels offers a range of surgical procedures for the face, breast, and body, but his primary area of expertise lies with body contouring procedures, especially for patients who have experienced significant weight loss.

Dr. Michaels has two practices in the DMV area: one in Rockville, Maryland, and the other in Fairfax, Virginia. He also welcomes consultations (virtually or in-person) with patients from other parts of the United States and abroad.

Patients who have lost a lot of weight—either through a healthy diet and exercise or with the help of bariatric surgery—are frequently left with sagging skin. Along with potential hygiene and health issues, excess skin can distort their body’s true appearance and make it feel as if their weight loss journey isn’t yet finished.

That’s where Dr. Michaels’s renowned body sculpting skills come into play, with a customized combination of skin removal surgery and body contouring. Procedures may include liposuction (SmartLipo, VASER lipo, or tumescent liposuction), tummy tucks, breast lifts, arm lifts, or a full upper and/or lower body lift.

Dr. Michaels received his medical degree from the Icahn School of Medicine at Mount Sinai Hospital in New York. After completing two residencies at the New York University Medical Center in general and plastic surgery, he was accepted into one of the few fellowships specializing in post-weight loss body contouring at the University of Pittsburgh Medical Center, making him a true expert in this field.

The depth of Dr. Michaels’s body contouring experience has made him an invaluable resource, often tapped as an expert by the media and scientific journals alike. He has been featured on CNN and Doctor Radio on SiriusXM and written more than 10 peer-reviewed manuscripts. He was the co-first author of the James Barrett Brown Award manuscript, an award presented to the best paper on a plastic surgery-related topic, published in the elite Plastic and Reconstructive Surgery journal in 2009.

Passionate about furthering advancements in plastic surgery, Dr. Michaels frequently performs research. He has been an investigator in FDA-approved clinical trials, most notably of tissue glue used in tummy tuck surgery (abdominoplasty).

He also fosters innovations in his field by directly supporting the next generation of doctors. In addition to being an assistant professor at Johns Hopkins School of Medicine in the department of plastic and reconstructive surgery, he helps train plastic surgery residents from Georgetown University and Johns Hopkins Hospitals.

Dr. Michaels has more than 12 years of experience as a plastic surgeon. In addition to being board certified by the American Board of Plastic Surgery, he is a member of two professional organizations: The Weight Loss Surgery Foundation of America and the American Society of Plastic Surgeons.

In his life outside his medical practices, he is a proud husband as well as a doting father to three wonderful kids.

7 months post op tummy tuck scar revision. Do I have dog ears?
dog ears
Feb 2, 2019
Thanks for your question. From the appearance in the pictures, you do not have dog ears. You have excess skin and some residual fat on your back that is causing your concern.
When performing an FDL, is it harder to construct a natural looking belly button (hooded, innie etc
Fleur-de-lis (FDL) and the belly button
Feb 2, 2019
Thanks for your question. I think that it is possible to get that “innie” belly button appearance even with a FDL incision. Because the belly button comes out the vertical incision, it is being held by the incision above and below the belly button. Until the scar softens completely, Hooding won’t likely happen.
Would I benefit more from an extended tummy tuck?
Extended versus regular tummy tuck
Feb 2, 2019
Thanks for your question. By just looking at your pictures, a full tummy tuck would suffice. An extended tummy tuck or 360 body lift only helps if you have excess skin around the hip area or the outer thighs/buttocks. I perform liposuction of the abdomen and flanks in nearly all my patients. Its old school thinking that liposuction and a tummy tuck can’t be done at the same time. You may have not found the right plastic surgeon for you.
What options are out there to help decrease the loose skin around the elbows?
elbow skin excess
Feb 2, 2019
Thanks for your question. First off, there needs to be some excess kin there when your arm is straight, otherwise you would be able to flex your elbow. That being said, sometimes liposuction alone can flatten out the area. In other cases, a direct excision of skin from that area can be performed.
Desperately unhappy with amount of asymmetry after breast lift. Can this be fixed? 7 months post br
breast lift revision
Sep 3, 2018
Given that you are seven months out, it is an appropriate time to consider talking about a revision. I think that your results can be improved with a surgical revision. You can always consider a second opinion from a board certified plastic surgeon.
Would a breast lift help me?
breast lift
Sep 3, 2018
There is something called a breast footprint. This is where your breasts sit on your chest and can be high, medium or low. In your case they happen to be on the lower side. The only way to increase the height is to add an implant and this may not be what you want. You do have some asymmetry of your breasts which is normal. This can be corrected with a lift on the left side. You can also consider some fat transfer to the upper and medial poles of your breast to improve volume and cleavage…
Should I get an inner thigh lift or a body lift?
inner thigh loose skin
Sep 3, 2018
A lower body lift will indirectly help your inner thigh skin. This is because your abdomen and pubic area contribute to the loose skin of the anterior and inner thigh. A lower body though will not correct all the laxity in the inner thigh and you will need an inner thigh lift to correct this. A lower body lift effects the outer thighs more. It also lift the buttocks. If the outer thighs and buttocks are not a concern then a lower body lift is too much surgery and an extended tummy tuck is…
Would a crescent or modified thigh lift give me good results or will I need a scar all the way to m
thigh lift
Sep 3, 2018
Awesome weight loss!! I think it depends on your goals to be honest. I will first say, I don’t think you need a vertical down to your knees. If you look on the posterior pic, you have a transition about half way where the loose skin stops in the upper thigh. I think if you want your thigh tight, a vertical scar half way down will give you the best result. If you’re against the vertical, then the crescent (groin crease) scar will remove loose skin and make the leg look better, but it won’t…
Why are my arm lift scars so bad?
arm lift scars
Sep 3, 2018
The arm lift scars are some the slowest to fade as compared to other body contouring procedures. It can take up to 2 years to fade and you are just at the one year mark. Scarring is mostly determined by your genetics, this is something that your plastic surgeon can’t change. Some of the other things you mentioned may contribute, but there is no way to know for sure. I would use silicone sheets and give it more time. Hope that helps.
Can I have an arm lift without an obvious big scar in my forearm?
arm lift
Sep 3, 2018
Since you appear to have loose skin and fat down to the level of your elbows, a full brachioplasty will be needed. It does not appear that you will need to extend onto the forearm. I do this routinely when necessary and the healing is often good. This will be patient dependent though, and mostly depends on the patient’s genetics.
Doctor’s suspect lymph damage but the plastic surgeon says it couldn’t be, any suggestions? (Photos
lymphedema and arm lift
Sep 3, 2018
Chronic swelling is a rare but potential complication of an arm lift. There are special tests that can assess the lymph drainage from your arms and it may be worthwhile exploring those options. Unfortunately, there isn’t much to do besides lymphatic massage, elevation and compression.
Will Insurance Cover Excess Skin Removal if It’s Causing Me Discomfort After Exercising?
body lift and insurance
Sep 3, 2018
Congrats on the awesome weight loss! Unfortunately, insurance companies consider a lower body lift cosmetic. If you have skin issues related to the hanging abdominal skin, sometimes a panniculectomy can be covered during the lower body lift. This will save you some money.
Lower body lift with 360 lipo or BBL, 360 lipo with thigh lift and breast lift/implants plus arm li
multiple procedures
Sep 3, 2018
Congrats on the weight loss!! Given the areas of concern that you have, I’d recommend two procedures. First a lower body lift and arms, second breasts and thighs. You want to make sure you’re at your goal weight prior to surgery. Ideally your BMI should be below 28, but 28-32 also acceptable. You appear to have plenty of breast volume so implants may not be necessary unless you want to be much larger that you are when in a bra. I think 4-6 weeks off per procedure is adequate.
Lower body lift last year, I still think I need the vertical component. The scar will not bother me
vertical skin excision
Sep 3, 2018
You will definitely benefit from the addition of a fleur-de-lis or vertical skin excision. The downside is the scar potential, but if you’re self conscious about the loose skin, then go for it!!

Flying after a lower body lift
Sep 3, 2018
Every plastic surgeon will have a different protocol for out of town patients. The biggest concern is blood clots. For that reason, I have all patient fly in 2 days in advance of their procedure. They also must stay at least 10 days afterwards for a lower body lift. I also put all of my patients on lovenox (blood thinner) following the procedure.
