Post acne marks. How can I get rid of them?
Treatment for post acne erythema
Jul 14, 2018
To treat residual redness that patient’s have from resolved acne I use the Vbeam laser from Candela. This an easy and effective way to treat the stubborn capillaries in 1-3 treatments.
Number of V Beam treatments for dilated capillaries and redness?
Vbeam for redness
Apr 6, 2017
Whether
or not your redness resolves or improves is dependent on the settings on the
Vbeam. More specifically, this is dependent on both the "pulse
duration" and "fluence". In order to improve redness when spider
veins are not present, a shorter pulse duration is needed in order to destroy
smaller vessels that aren't visible to your eye. A word of warning, when
shorter pulse durations are used, more bruising can take place.
Can Finasteride cause more hair loss?
Finesteride (Propecia) and hairloss
Apr 6, 2017
There
is no reason why it would worsen your hair loss. Rogaine on the other hand can
cause a temporary increase in hair loss in the first month or so. Propecia does
not have this side effect.
"Lip Pop" injection causing a speech impediment and cannot eat or swallow correctly.
Botox effect
Apr 6, 2017
Unfortunately,
I think your only treatment is time. Over time your botox effect will diminish
and your paralyzed muscle function will return and all will be back to
baseline. This may take several months though. In the interim, other muscles
may compensate and help the situation. Additionally, you may need on a short
term basis to adjust your eating style to compensate for your temporary motor
loss.
Would Candela Smooth beam laser work at reducing pimples?
Smoothbeam for acne
Apr 6, 2017
Smoothbeam
laser could provide you some improvement but if your acne is primarily hormonal
in etiology, there are better options: correcting any hormonal abnormalities,
oral contraceptives, and spironolactone are just a few such therapeutic
options.
Tumescent liposuction? Layman's terms please.
What is tumescent lipo
Feb 9, 2017
“Tumescent” means to make rigid
or distended with fluid. In Tumescent Technique Liposculpture, the fat space is
distended and made firm by the infusion of large amounts of dilute local anesthetic
fluid. This massive infusion of fluid into the fat space has many benefits. It
provides tremendous anesthesia that will typically persist for 12 or more
hours. This allows the procedure to be performed without sedation or general
anesthesia in the outpatient setting. This makes the procedure dra...
Why did I get broken capillaries after pulse dye laser for rosacea?
Therapeutic treatment
Feb 9, 2017
I have been using the Pulsed Dye
Laser for over 25 years and have never seen this happen. My guess is that you
are being undertreated. A sub therapeutic treatment could irritate and inflame
your Rosacea causing it to worsen and stimulate new vessel formation. A certain
energy threshold is needed for a laser treatment to be successful. Being too
gentle can actually have the opposite intended effect. This is commonly seen in
laser hair removal. When the laser energy is sublethal, the hair...

Treatment for Rosacea
Feb 7, 2017
Your best option is treatment
with a Pulsed Dye Laser. Ultimately, the problem is vascular. You have a poorly
regulated and excessive blood flow issue of the facial skin. The best means of
reducing this excessive facial cutaneous blood flow is by reducing the
cutaneous vasculature with the Pulsed Dye Laser. Rosacea medicines tend to
improve the vascular components of
Rosacea minimally. I generally see approximately 50% improvement in the
first treatment, 70-80% in two treatments, and...
Melasma. What can I do?
Proper treatment for Melasma
Feb 7, 2017
The first issue is if you have
the right diagnosis. Melasma is not typically unilateral. Assuming you do in
fact have melasma, the first order of business is meticulous sun
protection/avoidance. A topical skin lightening medicine should be a part of
the plan. I do not think IPL is a good treatment for melasma. It frequently
worsens the problem due to stimulation of rebound hyperpigmentation. I never
use IPL on melasma. The risks outweigh the benefits. My treatment of choice is
a Q-switched...
Should I try Botox or surgery for my hands?

Safe, easy, and always effective. You are likely to get 6 months of complete
relief with each treatment.
Can keloid scarring be avoided after ear lobe reconstruction?
Ways to prevent keloids after surgery
Feb 7, 2017
The best way to avoid post
operative keloids is to inject the surgical scar with cortisone. The first
injection should be 3 weeks post-op. This should be continued monthly until
there is no evidence of scar thickening or inflammation. This could be 4-6
months. I regularly excise ear lobe keloids and if this post-op regimen is
adhered to, I have never experienced a keloid recurrence.
I'm worried about how many units of Botox the clinic wants to use. What should I do? (35-45 for
Botox dosing
Feb 7, 2017
I generally treat the glabellar
frown lines (“elevens”) with 40 units on men and 20 units on women. Men
generally need a higher dose than women. Nonetheless, it is easier to add more
units if needed then it is too take it away (only time takes it away). My
approach with a new patient with trepidation and concerns about looking over
treated is to start at 20 units and instruct the patient to follow up in 2
weeks for a touch up if too much movement persists.
On average, do patients have a lesser breakout than the first time or is it about the same as t
Accutane treatment plan
Feb 7, 2017
I find that patients definitely
break out far less on their second round of Accutane. Very few of my patients
require a second round however. Approximately 95% of my patients get a long
term cure with their first round. The key is in the total cumulative dose of
isotretinoin (Accutane) they receive. If the dose is not high enough, the
likelyhood of an incomplete cure is much higher. In that your acne flared with
isotretinoin, I would start you on a low dose, allow you to acclimate,...
Liposculpture with hernia patch?
Hernia and liposculpture
Jan 28, 2017
Thank you for your question and researching the advice of other physicians.
I am extremely cautious in performing liposculpture in any patient who has a history of a hernia. The risk of an inadvertent bowel perforation is significantly greater in this scenario. A cautious approach is correct in this setting.
