I cannot over-emphasize the importance of checking doctor's training and board certification before you make a decision for any type of medical, surgical or cosmetic care. Dermatologists, plastic surgeons, facial plastic surgeons and oculoplastic surgeons are the only types of physicians who are trained in residency to care for this and similar problems. My main recommendation is to see a board certified dermatologist who has several tools/devices to help. You do not want someone who is not well versed in acne treatments, since some acne scar therapies can temporarily flare acne. You don't want someone who just does micorneddling, or just IPL, or just Fraxel. This is because you likely will need several devices over the course of several months to get you as clear and smooth as possible. Good luck.

Fillers into the forehead are usuually placed on the periosteum. This can cause pressure and result in pain. I always warn my patients about this complication. Typically, when it occurs, it lasts a day and is relieved by ibuprofen. If the pain is shooting and not pressure, it might indicate a nerve got dinged during injection, but that would have caused a sharp, quick and usually transient pain during injection. Why do my tear trough fillers not last more than 6 weeks?
Juvederm often lasts years in the tear trough, so this is quite odd.

I love Juvederm. But, Juvederm is my least favorite HA filler for the tear trough area, mainly because of its well-deserved, notorious reputation for delayed and persistent swelling. All HA fillers last around a year in this less mobile area. Your history is quite unusual, so I would seek a second opinion, optimally with an oculoplastic surgeon.
Microliposuction to remove Bellafill?

What a shame that Bellafil was used in this area. Bellafil, if used at all, should be injected in thicker skin and deeply. Even when injected as it is recommended in acne scars, I have seen long standing nodules. 5FU might be worth a try. IL TAC does indeed cause fat atrophy and indentations, especially in this area. Most people have to have them excised. I am so very sorry.
Can Sculptra enhance facial fat transfer? Or will it be detrimental to fat cell survival?

The most correct response is that the answer is unknown because Sculptra has never been studied and proven to have this effect. That said, the collagen-stimulating effect of Sculptra might enhance fat survival by providing a more robust dermal environment and thicker, healthier skin. 
Permanent lump after Botox 1 year ago. Does anyone know what this Dr could have done to me?

This is most unusual. In over 22 years of injecting Botox, I have never seen this problem. It causes me to wonder if something unusual was done. I would want to have more much more information than can be provided on this platform. If the doctor who injected cannot explain/fix the problem I would go to a board certified dermatologist or plastic surgeon in your area who does a lot of injections and get a second opinion. The most accurate answer is there have been no comparitive studies. Both tretinoin (Retin A) and tazarotene (Avage) have been FDA approved for photoaging (and collagen improvement). Adapalene has never been studied by Galderma for this indication, but might help. In my clinical experience, adapalene is not as good for aging/scars/collagen building.
Should I keep taking Doxycycline if I don’t have acne anymore?

Even in more severe acne, it is not a good idea to stay on antibiotics for more than a few months. After that, topical medications should be able to maintain the clearance. If not, Accutane, hormonal therapy, or treatments like laser to suppress the over-active sebaceous glands should be considered. Discuss all your options with a board-certified dermatologist.
Is this normal after Botox?
Bruising is the most common complication of Botox.

Even with a tiny needle, a blood vessel can be pierced.  Bruising is much more common if you are on aspirin, Vitamin E, and many herbal remedies. The bruise is immediately blue, and then turns brownish-yellow as the hemosiderin is metabolized. There is no reason for concern.

You had impetigo. Perhaps an acne lesion was picked? Or you were exposed to a child with impetigo and touched cheek to cheek while you has an outbreak? Somehow you got a bacterial infection.  In any case, the crusty area that is now discolored was likely impetigo . The erosion has left you with discoloration. Your dermatologist can correct this new problem.

You have darker skin, so anything other than a very light peel can be problematic. If an erosion like this, so early on (it is best for the skin to flake, not slough off) occurs, it could put you at risk for post-inflammatory hyperpigmentation. When identified and addressed early, it will be a non-issue. But if treatment is delayed, the dark areas at the site of the erosion can last months-years. Your dermatologist can easily fix this, so get ahead of the problem.
Masseter Botox. Will it help give my face a round shape?
It will not. It will make if more oval or angular.

The fact is, Botox in the masseters will give your face a more oval or angular face by reducing the lateral jaw bulking of the masseter. This is preferrred for femininity more than a square or round face. Aggressive old fashion ablative dermabrasion, Fraxel Restore, Microneedling, radiofrequency microneedling, subscision and dermal filler are options. There have been recent studies to suggest we are being overly cautious with the wait period post-accutane for resurfacing. For sure, dermal filler with subscision can be done now because no re-epithelialization is needed for healing of those 2 procedures and in tandem, they get better results than either alone.
