From the photograph, it looks like that your eye bags are mostly a result of hollowing rather than excess fat. You would likely benefit from non-surgical treatments such as hyaluronic acid fillers to add volume in the area of the hollowing. I would recommend that you have a consultation with an oculoplastic surgeon who is familiar with the anatomy and injecting fillers in this delicate area.

Botox injected into the frontalis muscle to treat forehead lines can cause the eyebrows to droop. You can see the maximal effect of the Botox typically at around 2 weeks after the injections and then the effect fades over time. It is hard to tell from the photo the amount of upper eyelid ptosis since the head is tilted and you are probably raising your eyebrows to try and pull up on your eyelids. In order to have ptosis surgery covered by insurance, the eyelids must be low enough to affect your vision which affects your daily activities. It sounds like you would like your upper eyelid crease to be a bit higher. In my opinion, it is usually best to be conservative and to not raise the upper eyelid crease too high and to not take off too much upper eyelid skin since it is easier to make the crease higher or to remove more skin than to put it back or lower the crease after the surgery is done. Filler can cause swelling especially in the delicate under eye area. I would wait at least 2 weeks for the swelling to go down. If you are still unhappy with the appearance of the filler at that point, it can always be dissolved. For filler injections in the delicate under eye area, I would recommend seeing an oculoplastic surgeon.

The left upper eyelid looks to have healed after surgery but there is some ptosis of the left upper eyelid contributing to the asymmetry. The brow on the left side is also a little raised compared to the right side possibly as compensation for the ptosis which accentuates the asymmetry of the upper eyelids. I would recommend either seeing your original surgeon or an oculoplastic surgeon for evaluation to see if the asymmetry can be improved.

Bruising can happen after xeomin injections especially in the delicate tissues and fine blood vessels around the eyes. Your chances of bruising are also higher if you take any blood thinners, NSAIDS, vitamin E, fish oil, or certain herbal supplements. The bruising tends to settle in the area right below the eye due to gravity and at this point applying warm compresses, IPL or laser treatment can make it go away faster.

It appears that you have some hollowing in the undereye area and this can happen at any age. You can consider adding volume in the area with filler or fat injections but it may be best to start with fillers first to see if you like the way it looks. There are certain fillers that I recommend for the delicate area under the eyes (e.g. belotero, restylane silk) and it would be best to consult with an oculoplastic surgeon who is experienced in injecting the area.

There may be some brusing and swelling initially after the Voluma injections. You can try applying some ice to the area but if the pain gets worse and not better, then I would recommend going back to see your physician. Otherwise, I would recommend waiting at least 2 weeks to see the result of the injections after the swelling has gone down before deciding if you would like any of it dissolved.

If you like the appearance of the lips when there was swelling and more volume than you may need more filler to achieve that look. I would recommend waiting at least 2 weeks for the existing filler to settle before going back for more filler.

If you are unhappy with the appearance of the top lip, the filler can be dissolved with hyaluronidase. Everybody’s aesthetic is a little different but a general rule of thumb is to aim for a 1 to 1.6 ratio for the upper to lower lip. You could return to the same injector to dissolve the filler or find another physician who is experienced with lip fillers.

A 30-35% TCA peel should help diminish the fine lines around the mouth. A combination approach is sometimes best with laser, microneedling, and filler injections. Also, try to sleep on your back if possible to prevent the formation of sleep creases on either side of the face.
