The amount of sagging skin after the Vline surgery depends on each patient.  Each of my patients wears a facial garment afterwards to help support the soft tissue and have it seal to the new bone.  I have found that in young patients with elastic skin this usually heals fine. I do offer patients the Renuvion (or Jplasma) skin tightening at the same time.  This is great for older patients, or young patients who have some skin laxity.  This can be done at the same time as Vline surgery, or af…

Yes!  There are several different ways to perform Vline surgery.  The surgery is catered to each patients needs.  Sometimes just bringing the chin down and forward creates a better “V”  Creating a steeper angle from the angle of the mandible to the chin is generally feminizing; and this can be done by changing the angle, the chin or both.  Of course, visit with a craniofacial surgeon to see what options best fit your face. Best of luckDr Rodman

Hello and thank you for your photos and question.Based on what I see in the photos, your chin is out of proportion to the rest of your jaw. I recommend a sliding genioplasty to augment the chin and bring better balance to the lower 1/3 of the face.You may want to consider buccal fat removal at the same time to help slim and narrow the cheeks. Botox to the masseter muscle may help some, but will not likely be a definitive solutionBest of luckDr Rodman
Do Kenalog injections after rhinoplasty reduce the swelling?

I use kenalog to prevent unwanted scarring.  It shouldn’t be used for swelling, but in some cases where there is excess fullness, this is scarring that is trying to form.  Kenalog prevents the scarring from forming and helps decrease the fullness in these areas.  This is why it is so important to follow up with your rhinoplasty surgeon.  I tell patients I can give about 10-15% better results if they come for post op visits. Best of luckDr Rodman

Sorry to hear you are going through this.  From your photos, it does appear your result is suboptimal. Having not been at your first surgery, I’m not sure why a septoplasty was not done during the primary rhinoplasty if you were having breathing issues.  But the good news for you is that if septoplasty was not performed, there may still be enough cartilage in your septum to correct your nose.  In these cases I usually consent the patient for a revision rhinoplasty with septoplasty and pos…

Unfortunately science and technology has not yet found a way to make the thickness of skin not a factor.  However, a good rhinoplasty surgeon knows how to work with each skin type and takes that into account when planning for surgery. The analogy I tell patients is “imagine a basketball under a sheet versus a basketball under a puffy down comforter.”  Under the sheet, one can see every detail of the ball.  Under the comforter, one may see a lump but it will be hard to make out the exact sha…

To answer your question, I would need to know, What is your goal?  What look are you trying to achieve?  If you want to fill out your jaw, implants are the best solution.  If you want to make your chin shorter, a reduction genioplasty is the answer.  If you want some combination of the two then perhaps both?I recommend consulting with a craniofacial surgeon, to get a thorough evaluation of your face and to discuss your goals and desires.Best of luckDr Rodman

From your photo it looks like you have a Class III occlusion, or underbite.  This can be corrected, but it will involve a few steps. First you should meet with an orthodontist and a craniofacial or maxillofacial surgeon.  You will likely need to be in braces, and then jaw surgery to move the jaw backward. For most patients the shape of the chin is determined by the bone.  To create a real change in the shape, the bone needs to be reduced or set back. It’s hard to see from the photos exactly what it happening but the answer is yes there are several options. Just laser treatment does not look like it will be enough. If you need a big advancement to your chin, skip the fat and the fillers.  The best way to move the chin forward is with an osseous genioplasty.  The shape of the chin is determined by the bone, and so moving the bone is the best and most effective way to change the shape of the lower face.  The movement created by a genioplasty is much greater than any fat grafting.See a craniofacial surgeon to discuss options.  Best of luckDr Rodman

First, I am so sorry this happened to you!Unfortunately, the location of this scar is a very difficult place to repair.  This is an area where there are many contours to the lip and everything needs to be aligned perfectly.  I would be hesistant to do any repair right away.  It looks like you have whats called a pincushion deformity, which is where lymphatic fluid is pooling in the scar.  This will likely go down with time.  You are doing everything right; scar gel and massage.  I would…

Good question, and the answer is…maybeThere are two reasons I break the bones in the nose.  One is because it is crooked and the other is to narrow the upper part of the nose. When I file down the bones that create a dorsal hump it often leaves that part of the nose wide and flat.  I break the bones to “cave them in” so the upper 1/3 appears narrow and straight.  In cases where I don’t need to file a large hump I prefer not to alter the bones.  However, most patients who have a large hump…
