My answer is try to “kick the can down the road” as long as possible. The limitation of hair transplant is there is a fixed amount of good genetic hair to transfer to the balding area. Therefore, my philosophy is to do as little as possible to meet the patient’s goal and keep them happy. If the preventative medical therapies are keeping my patient happy, why do a hair transplant? By “kicking the can down the road” we are also buying time for better medical answers.
I am thinking about a hair transplant, is this the right decision for me?

This is answered by the concept of “donor dominance” in hair transplant. The transplanted hair will keep the same characteristic from where you took it from. If the hair is curly, it will stay curly. If the hair is fine, it will stay fine. If the hair is white, it will stay white. Etc.

Since there is a limited amount of transplantable good genetic hair, my philosophy is to do as little as possible to accomplish my patients goal. Therefore if their goal can be accomplished with the preventative medical therapies (i.e.- finasteride and minoxidil) than that is all I would do. I place approximately 20 grafts per cm2. If that area is 40cm2 than the number of grafts would be 800 grafts.
