Based on your photos a softer filler may be most appropriate for these smile lines. These can be injected superficially under the lines to soften them. Botox is not the appropriate treatment for puffiness in the tear trough areas. However, fillers may be used by an expert injector to correct hollowing and puffiness in the tear trough area. Eyelid surgery is also an option. However, I advise consulting with an experienced cosmetic provider for consultation regarding correction with fillers first. Good luck!

Isotretinoin is the most effective treatment for moderate to severe inflammatory and cystic acne specially if resistant to oral antibiotics and topical regimen. Isotretinoin also provide long term response and which is specially important in older patients. Both Accutane and topical Retina-A are vitamin A derivative and thus if used together their side effect are additive. Specifically the dryness, irritation and sun sensitivity caused by Accutane can be exacerbated significantly if one uses topical Retin-A at the same time. Therefore, I recommend avoiding use of Retin-A while on Accutane and for at least one month after completion of the Accutane course.

Since B5 is not a Vitamin A derivative it it is okay to take while on Accutane. You should not take any Vitamin A or it’s derivatives while on Accutane.
