How do you know if you have a brow droop or if you just need a little more Botox around that eye
brow droop
May 8, 2020
Thank you for your question. If you are trying to determine if the Botox injection has affected your brow position, ultimately you will have two options. First, depending on when you were last injected you could wait 3-6 months for the effects of the Botox to wear off and see if you notice much change in your brow position. Alternatively, you could ask your injector to inject a small amount of Botox under the brow (into the orbicularis oculi muscle), this can often help improve the ...
What to do about bad results? Under eye fillers seem to have made my under eyes look worse. (pho
under eye filler
Mar 14, 2020
Thank you for your question. If you notice an elevated area immediately following your filler injection it is sometimes possible to gently massage the area and it will even out. If you are two months from your injection and dissatisfied with your results, assuming you were injected with a hyaluronic acid based filler (Restylane, Juvederm, etc) the filler can gradually be reduced with injections of hyaluronidase. I would recommend discussing hyaluronidase with your injector.
Is it okay to do lower and upper separately or will the lower blepharoplasty cause my upper eyel
Upper and lower eyelid surgery
Feb 10, 2020
Thank you for the question. You could most certainly have a lower eyelid blepharoplasty without having an upper eyelid surgery. Your lower eyelid surgery should not make the uppers sag more, most lower eyelid surgery is done through the inside of the lower eyelid to address the fat prolapse and volume loss and will not affect the upper eyelids. If you have dry eye, make sure that your surgeon is conservative in the upper eyelid surgery, one of the potential complications of blepharoplasty is...
I'm noticing dynamic wrinkles under my left eye only. What is the best treatment?
dynamic wrinkles
Jan 26, 2020
Thank you for your question. If what you are trying to improve upon are the wrinkles that you notice with animation (when you are smiling) your best bet would be neurotoxin (Botox, Xeomin, Dysport). As you mentioned over done neurotoxin can give an unnatural appearance, have your injector start with a fairly low dose of neurotoxin (you can always add more if you did not get the desired effect). CO2 laser would work well for fine lines around the eyes that are present at rest. Filler is a...
Can you help me after getting a blepharoplasty under my eyes made my issue worse?
Lower eyelid festoons
Nov 24, 2019
Thank you for your question and photos. If you are looking for a non-surgical option, I would recommend an injection of doxycycline into the area. You can get some increased swelling in the initial 1-2 weeks following the injection and you may need to have the injections repeated but I suspect if you had 1-2 injections of doxycycline possibly followed by a CO2 laser you would have a fairly nice result. These can be addressed surgically as well but make sure your surgeon has handled these...
What caused the distorted bag under my right eye and the severe wrinkling when smiling? What are
Lower eyelid wrinkle
Oct 5, 2019
Thank you for your question and photos. There are a couple things that I am noticing in your photos you have supplied. If your main area of concern is the wrinkles that you notice when smiling, that appears related to what is called the orbicularis roll and can be treated in the office with neurotoxins such as Botox. If you are also bothered by the lower eyelid/mid face appearance at rest, you do have some volume loss in the tear trough area as well as some fat prolapse. If you are looking...
What procedure can lift the ends of the eyebrows without committing to a full brow lift?
Temporal brow lift
Sep 16, 2019
Thank you for your question. You will actually have several options. First, if you have not already tried it, a neurotoxin such as Botox can be injected just under the brow which can get you a small amount of lift. If you do not get the result that you would like with a neurotoxin or are looking for a more permanent solution in my opinion your two best surgical options would either be an upper eyelids blepharoplasty in which a small strip of the orbicularis muscle is removed just below the...
Is lateral canthoplasty revision possible?
Canthoplasty
Sep 14, 2019
Thank you for your question. Did you have the canthoplasty to revise a previous surgery or was it to improve a natural asymmetry? It is not uncommon to develop scarring in the middle lamella (the area between your orbicularis muscle and the conjunctiva and sometimes a spacer graft in necessary to adequately elevate the lower eyelid. While you may ultimately need a revision, there are a variety of things that can be tried to improve the way in which the eyelid heals. Both triamcinolone...

Endoscopic vs temporal forehead lift
Jul 20, 2019
Any incision that is placed in front of your hairline (pretrichial incisions) can act to shorten the forehead. In deciding between a temporal pretrichial forehead lift and an endoscopic forehead lift, I think the real question is how much lift are you looking to gain in the central half of the brows. Many surgeons will incorporate a temporal excision into their endoscopic brow lifts. The advantage of a temporal pretrichial brow is that it can often be done under IV sedation with less...
Can dermal fillers or Botox fix upper eyelid retraction caused by TED?
Eyelid retraction
Jun 18, 2019
To answer your question, yes, a filler can be tried on the left side. The challenge with using filler is that you need to try to get vertical elevation without adding bulk to the eyelid (which would look worse cosmetically). For eyelid retraction associated with thyroid eye disease your better option may be surgical repair of the retraction (even if you have to do this cosmetically)
Under eye bag on one side when smiling. Should I have lower blepharoplasty?
eyelid bag while smiling
Jun 9, 2019
Surgery can be a great option for static changes in the lower eyelid/midface (when your face is at rest). Surgery would not help you with a dynamic rhytid (wrinkle) such as this. It appears that you have a little more orbicularis tone on smiling on the side that is bothering you. A neurotoxin injection (Botox, Xeomin, Dysport) would be a great minimally invasive way to improve this.

May 21, 2019
I suspect that this is all from swelling in the upper eyelids. So long as things are not worsening this is likely a post surgical change. You will find the the eyelids will improve over several weeks. Do not hesitate to contact your surgeon if you feel like things are starting to worsen though.

Bags after filler
May 8, 2019
Thank you for your question and photos. In the lower eyelid area there are three main changes that happen with age, volume loss (for which filler is great for), age related skin changes (increased laxity of skin with fine crepe like appearance) and orbital fat protruding forward. Fillers will only address the volume loss and all fillers tend to cause swelling my drawing fluid to them. I suspect that in the last 5 years you have had some increased laxity to the skin of the lower eyelid and...

asymmetry after trauma
Apr 30, 2019
Thank you for your question.
There is a lot that can possibly be done for you and it is possible that not all of the procedures you would need would be considered cosmetic. Asymmetry following orbital trauma (most commonly enophthalmos) is a fairly common occurrence if the original trauma was either not repaired or inadequately repaired. The main question I believe in your case is why is there asymmetry. Is it due to silent sinus syndrome or possibly a fracture of an orbital wall. I...

Congenital Ptosis
Apr 29, 2019
Thank you for your question and the supplied photo.
Congenital ptosis, which is what it sounds like you were born, can be a very difficult condition to treat. The reason for this is that in most cases of adult ptosis there is a normal eyelid muscle that has simply stretched over time and by advancing the normal eyelid muscle you can achieve quite a bit of improvement and symmetry compared to your contralateral side. In congenital ptosis, the levator muscle in the upper eyelid is not fully...
