You could try one of the restylane products like kyss or define or juvederm volbella Or belotero

These are tough to fix and the angle of the photo is not ideal
However, it looks like you have some banding of the platysma muscle. Depending on your age and the condition of your skin, these are usually easily treated with Botox, Xeomin, Dysport or Juveau.If that does not help, surgery done by a plastic surgeon or facial plastic surgeon can tie the bands together and improve the appearance.

There are so many options to consider and surgery is, to me, something to use down the road.For starters, I would use a pulse dye laser (not an intense pulsed light) and treat the area a few times. I would consider using a fractionated erbium laser after that. If that did not work, I would inject very dilute steroid into this area (diluted because it can cause the skin to get too thin). It may do a few things both positive and negative. The positive is that hyaluronic acid injections (restylane) have been shown to stimulate your own collagen so this can produce some degree of long term correction.

The product is pretty cohesive and usually stays where it is put.While there is no data to tell us how long it takes before it can not be moved easily, it is likely that the product is still there. Looks like your smile is uneven because of some issue with one of the muscles that pulls the corner down. This may be congenital or acquired. To counter this, it is reasonable to inject Botulinum toxins into the opposite side to balance the smile.
With respect to the TMJ, I have been having a great deal of success injecting 40 units of Botox or about 100 units of Dysport into the jaw muscles. For some people, I follow this with injections into the temple region to calm that muscle as well.

Great questions. Fortunately, the risk of biofilms is low - so low in fact that there is not enough data to know the answer for your question because there are not enough people that develop them. However, I would tend to avoid injections when you are having a flare and I would routinely take antibiotics for a few days around the time of your injections to minimize the risk of an outbreak.

I think that either of these will be good for this area. I dont think there is any duration data to decide which one will last longer.
Defyne is designed for deeper volume loss areas so will probably be better for deeper tear troughs while Refyne is better for fine lines and wrinkles so probably will be best for mild tear trough deformities.
