What could cause a scar to be resistant to Vbeam laser? What else can be done?
Scar revision
Aug 2, 2022
I suspect there is an element of pigment and I would recommend fractional 1927 to help remove pigment (hard to tell without examining closely).
Bottom-heavy face/ protruding - where should I place fillers? What other minimally invasive pro
Facial Contouring
Jul 9, 2020
Botox can be placed in the masseter muscle to weaken the muscle and contour the lower face. Treatment with Botox to the masseter muscle is also ideal for patients who are grinding and clenching their teeth.  Over time this can create a very squared lower jaw, contributing to a more masculine appearance and fuller face. Botox will relax and thin the muscle, which would result in thinning of the face.  You would also benefit from filler to the upper lateral cheek and temple areas to improve shape.
Is Sculptra injected with a cannula or a needle?
Sculptra
Jul 9, 2020
I prefer to inject Sculptra using a needle, as I feel it does not inject well via cannula. I also prefer to use hyperdiluted Radiesse injected more superficially in the neck, verses Sculptra.  This is not to add volume, it is to provide skin tightening, it acts primarily as a biostimulator to address concerns related to skin texture, fine wrinkles, and crepiness.
I have Crohn's and Glaucoma can I get Botox injections in my forehead?
Crohns - Glaucoma - Botox
Jul 9, 2020
It is safe to receive Botox injections with Crohn's Disease and Glaucoma.  Botox is injected in small targeted doses to reduce the appearance of fine lines and wrinkles (this occurs by weakening the underlying muscle causing the contraction).  When used for aesthetic purposes, Botox is slowly metabolized by the body, making it safe for cosmetic treatment.
What can I do for my heavy mouth corners, not marionette lines? I’m hoping for something that w
Treatment for DAO
Jul 9, 2020
I would advise a neuromodulator injection (such as Botox) to the DAO (Depressor Anguli Oris muscle). Neuromodulators block signals from nerves to the muscles and because of this the muscle is unable to contract. This is often subtle when done alone. If combined with a filler, it will have a more dramatic effect. Filler will help restore the loss of volume.  The results can be fairly long lasting, typically 9-12 months.
What procedure do I need? Advised that removal of upper eyelid skin will increase hollowing. (p
Non-surgical options for heavy inner eyelid treatment
Jul 9, 2020
I think you would benefit from a neuromodulator (such as Botox) to help elevate the brow and eyelid area. This produces a similar effect to a surgical option without the use of invasive surgery. I would also advise the use of a filler in the lateral brow area and possible temple to enhance contour.
What safe facial would you suggest for darker pigmented skin around my mouth and nose?
a gentle and safe solution to even skin tone
Apr 6, 2020
A good place to start is with a good skin care regimen that includes a topical depigment agent, antioxidant protection, and daily sunscreen.  This regimen should include or more skin care products that contain a tyrosinase inhibitor that will slow down melanin production result in lighter skin.  Tyrosinase inhibitors include hydroquinone, bearberry extract, azelaic acid, Kojic acid and Vitamin C.\

Photodynamic therapy as an option for early rhinophyma, advice?
PDT for rhinophyma
Apr 5, 2020
Treatments for rhinophyma include all the typical treatments for rosacea - antibiotics, medicated creams and cleansers, gentle skin care with good sun protection, and even isotretinoin. Vascular lasers can be used to treat the broken capillaries (blood vessels) common in all forms of rosacea. Some lasers target the over active oil glands - Smooth Beam and photodynamic therapy. However, often in rhinophyma, the tissue enlargement get so significant that these treatments are not effective.

enhancing results from PDT
Apr 5, 2020
It can be OK to not have much all over redness with a photodynamic therapy (PDT). However, typically the sundamaged, precancerous lesions on your face should be at least a little red and scaly or crusted. There are a variety of things we do in our office to help enhance the effectiveness of the PDT  treatment.  Before coming in for a PDT treatment, we often advise - especially in patients with widespread actnic/precancer lesions - to pre-treat the areas with a topical chemotherapy agent lik...

Inflammatory bumps
Apr 3, 2020
These bumps look like inflammation of the hair follicles, called folliculitis, which has a variety of causes - potentially related to mechanical irritation/sweat/occlusion. I recommend seeing a dermatologist to help with treatment of active rash and prevention of further inflammation, as there are topical medications and other interventions that may help. If redness persists, a laser called Vbeam may be useful to fade the redness. Good luck!
Post PDT Blue light with Levulan exercise?
exercise after PDT treatment
Apr 3, 2020
I think you can exercise almost right after PDT as long as it is indoors and not near windows. In general we advise avoiding outdoor activities for 48 hours as the sun exposure might make your PDT reaction more severe. there is some debate that this more severe reaction may enhance results depending on how much actinic damage you have on your skin -if you have a lot of sun damage then a robust reaction might be very good - may be inconvenient for you though with increased redness and ...
TCA Cross Post 4 weeks - Photodynamic therapy for acne safe?

Apr 3, 2020
Yes that would be safe. PDT is a great treatment for acne so hopefully it will bring your acne under control very soon!
Can I do Photodynamic therapy 24 hours after Botox?
Botox and PDT
Apr 3, 2020
I would not combine PDT for acne and botox neuromuscluar relaxer injection on the same day but the next day would be fine.
Can I have photodynamic therapy if I have filler in my face?
PDT safe after filler

Yes - you can have photodynamic therapy probably as soon as one week after filler in the area.

Eye area hollows
Apr 3, 2020
I love Pelleve for tightening the skin around the eyes. Great for tightening and smoothing. Your indentation looks like a natural hollow that commonly becomes more prominent with age. A dermal filler such as Restylane or Belotero can be used to correct such contour changes and irregularities.
