I would suggest starting with HA fillers. Those are the safest and most widely used fillers. I strongly recommend that you do not use Bellafill as this is a permanent filler with higher risk.

You could try another brand of botulinum toxin such as Xeomin which may have a lower risk of resistance. Xeomin is ultra-purified to remove the accessory proteins which are bound to the toxin. These proteins are not necessary for therapeutic efficacy but increase the chance of developing antibodies to the product. Resistance to Botox has no bearing on your response to fillers.

I would need to see you in person to give an exact recommendation, but you would likely benefit from filler to the mid-face (anterior cheeks and possibly tear troughs) as well as filler to the lower cheeks. Filler that I like to use include the Restylane family of hyaluronic fillers as well as Radiesse or Sculptra. Our goal with fillers is facial rejuvenation and volume restoration while maximizing safety and providing natural-appearing results.

You would likely benefit from use of fillers to the cheeks and jawline. This can add volume to sunken areas and tighten the skin in a non-invasive manner. You might possibly benefit from a filler such as Radiesse injected under the skin to provide skin tightening. Make sure to see an experienced injector.

You could likely consider conservative filler in the tear troughs. With proper technique, filler injection is typically non-invasive with little downtime or recovery. This is an advanced area and it is important to see a very experienced injector. I prefer to use a blunt cannula rather than a needle under the eyes. This reduces the risk of bruising or side effects and gives the smoothest results in my hands.

If the vein is outside the orbital rim, it is likely treatable with long-pulsed Nd:YAG laser. This is typically non-invasive with minimal recovery. I have successfully treated blue veins under the eyes with laser. However, I do not treat if the vein is inside the orbital rim. It requires an in person evaluation to palpate the underlying bony rim to determine the exact location of the vein in relation to the structure underneath.

Thank you for submitting such excellent photographs. In my hands, I would suggest starting with fillers to the cheek hollows below the area of the edema as well as in the tear trough. (Restylane Lyft and Restylane). I would start conservatively and add more over time to try to achieve the desired result. I would stay in a deep plane to avoid causing any puffiness. I have used Restylane for years in the tear trough with a very low incidence of problems. I have also used Belotero successfully here. I would avoid Juvederm Ultra in this area because it pulls in too much water. The key in my hands is to use a blunt cannula for tear trough filler. This makes treatment safer and minimizes bruising.

Please see a cosmetic dermatologist or plastic surgeon with extensive experience treating scars. Fractional lasers would likely help. Consider use of topical silicone gel products. Botox would probably be beneficial for the long-term. You need to see an expert physician in person who can direct your care. Generally scars can be improved but it is a slow process which requires multiple treatments using multiple different treatment modalities.
