The vertical aperture of that eye is smaller than the other and you can see that the light reflex on the cornea is closer to the edge of the upper eyelid on the droopy side. When you smile, the ring muscle around the eyes bunches up and that bunching gets transmitted to the overlying skin. This is what causes the strong lines under your eyes with smiling. You can try a small dose of Botox to the central lower eyelid to prevent the muscle from bunching as much. A longer lasting solution would be to have a lower eyelid blepharoplasty and have a conservative amount of skin and muscle resected so the area is less redundant. Good luck!

It appears from the photo that you have mild ptosis (drooping) of the left upper eyelid. This can be corrected by tightening the tendon of the muscle (levator) within the eyelid. This can be approached either from the front of the eyelid (lid crease incision) or from the underside of the eyelid. Your surgeon can test you with drops in the office to see if you are a candidate for the latter procedure. This condition will slowly worsen and affect your vision more. Good luck!
