Thanks for your question! Haivng a nonsurgical rhinoplasty before a surgical rhinoplasty can create problems during the surgical procedure, but those complications are rare. In a patient that has had filler to the nose before, the primary concern when undergoing a rhinoplasty is compromise of the skin envelope if the filler was placed too superficially (close to the skin). Rib cartilage warps to varying degrees primarily based on calcification of the rib which increases as you get older. In younger patients there are very few calcifications so the rib warps a lot. It’s difficult to make a full recommendation based on the photos, but it appears that you may have some volume deficiency of the lid-cheek junction which can be improved with both filler and fat transfer. But I would definitely tailor the recommendation based on an in-person exam of your eyes and the area around the eyes. Personally I would do both of these simultaneously. Restoration of volume at the same time as removal of excess skin and re-supporting the deeper layers of the face is a powerful procedure that can give an excellent aesthetic result.
