I would discuss with the surgeon you chose to provide the treatment. It depends on where the piercing is and where the filler is to be placed. You do not want to introduce bacteria into the filler as it can form a “biofilm” and a difficult problem. Hope this helps

A lip lift is a great option for increasing the red lip and dental show, while decreasing the height of the white lip. It is a relatively simple procedure but requires meticulous attention to detail from the surgeon. There is a scar just under the nasal sill/base, but it should not be noticeable in most situations. This is a very common concern, even in young patients like yourself. The hallowing below the eye is called a “tear trough” and forms as we get a little older. The shadow caused by the depression causes the dark circle to appear. A soft hyaluronic acid filler is a great option to correct this. Sometimes there are also pigment differences in the lower lid skin.

Great question! It is a little hard to see from the photos what changes might have occurred. Laying down, sleeping etc can cause temporary changes in the appearance of your face but will not cause permanent changes. However, our face does change as we are or gain/lose weight. For augmentation of the cheeks and jawline/chin, fillers are a great place to start. There are more permanent options but I would recommend starting with fillers. I agree with your assessment. While it is attempting to consider fillers in this situation I would be cautious about using fillers in the tip of the nose after a rhinoplasty. If you want to consider this option I would discuss with a surgeon who performs surgical and non-surgical rhinoplasty. As for a revision, you may be able to get an endonasal (closed) revision with placement of a small tip graft for better definition through your thicker skin. Good luck!

Great question! A full examination would be helpful to address the areas of concern. One common complaint is that of dimpling of the skin in the chin. This is typically due to over activity of the mentalis muscle and can be addressed with botox to that muscle. Hope this helps!

I would recommend seeing a surgeon who performs cleft lip and palate surgeries as a full examination would be helpful. Minor revisions can typically be done to address the lip if needed. Of course, I always recommend to my patients to cease smoking! Good luck.

As we age we all lose volume in our face. This process is accelerated in some and can be affected by behavior to some degree. Most often, I will see avid runners have an exaggerated early loss of facial volume which is reflective of overall low body fat. Sometimes regaining weight will help, but we often see that facial volume changes independent of overall body weight. There are some options to help with thick skin but most topical and light treatments are modest in their benefit. I suspect a rhinoplasty would be helpful in your case. There are specific techniques for addressing thick skinned individuals to project and refine the tip of the nose. Not infrequently, a reduction in the size of nostrils can be beneficial as well. I would recommend you see a surgeon who offers both non-surgical rhinoplasty and surgical options. An examination is key. Based on your photos, I think you would have a very modest amount of improvement with filler alone and if you were my patient I would suggest that you consider surgical options before investing in filler that may leave you underwhelmed.
