Breast implant exchange - 550cc or 600cc?
implant exchange
Sep 2, 2020
Based on your photos, and just limiting the answer between those two sizes, I would say that you are likely to be happy with either size, really just being a difference of only about 10%. But a jump from 400 to one of these sizes is about a 50% increase in size, which may be too large for your own anatomic dimensions, as well as the significant increase in weight, which will cause your breasts to sag or droop. I would keep these factors in mind more than the trying to decide between the...
Why can’t I get lipo on my inner thighs? When I asked a local dr they said it wouldn’t work beca
lipo inner thighs
Sep 2, 2020
A full in-person evaluation and photos can help me determine if I would recommend liposuction as a primary means to reduce the bulk between your thighs, and your doctor may have been correct. That being said, plastic surgery is both an art and a science, and this doctor may not feel he can achieve your expectations. You may find others who will offer you this procedure based on their experience, so it may be worthwhile to get a few opinions. But I would caution you to not forget your...
Lost 95lbs with weight loss surgery. Arm lift vs lipo? Tummy tuck, mini tummy tuck or lipo? (pho
weight loss
Aug 11, 2020
Based on your degree of weight loss (95lbs), a skin removal/excision procedure is generally the solution rather than liposuction, which relies heavily on the skin being able to retract adequately to give you a great result. Only an in-person consultation can answer this question for you definitely, although the pictures help. But based on your photos, I would have to lean toward my recommendation above. Best of luck.
There is a hump on my shoulder that feels hard to touch. What does this look like and what kind
Lump on shoulder
Aug 1, 2020
That is very difficult to see on your photos, which is why an in-person consultation is needed to help make a diagnosis. Since this may be a cosmetically-sensitive area that is exposed, I would start by calling a plastic surgeon, who does not only do cosmetic surgery but also a fair amount of reconstructive surgery. This may be covered by your insurance. Best of luck
Does it look like I have snoopy deformation? If so, how can I rectify this issue?
snoopy dog deformity
Jul 27, 2020
Based on what I see (only a live consultation can really determine completely), while you have a snoopy dog deformity, it appears that you have some capsular contracture in addition to inadequate treatment of breast ptosis (drooping), which really necessitates a lift in addition to augmentation. Minor ptosis, and snoopy dog deformities, can be improved with a dual plan technique, but significant ptosis can not be corrected with a breast augmentation alone.
Is it possible to get your dorsal hump shaved without breaking the nose if the bump is very slig
small dorsal hump
Jun 30, 2020
I would want to know what your fears are about "breaking" the nose, which we call lateral osteotomies. I do think that it is possible with the fairly minimal dorsal hump that you have, but the top of the nose would end up looking more square.  I think your concerns are very common, but I think it is better to frame the question as, what do you really want as your final outcome?  Plastic surgery is generally not the place to compromise, and there has to be a certain amount of trust in the pro...
Can I have a breast augmentation without a lift? I would like them to look natural
Breast augmentation without a lift
Apr 8, 2020
The great thing about plastic surgery is it becomes a balance between art and communcation, so you get get different answers from different surgeons.  You will have to trust your own gut when it comes to the surgery that is right for you.  Looking at all the answers, one option to consider is to have an augmentation without lift and expect that you may need to do a lift later if you feel that the implants were not enough to give you the lift and projection you desire.  We all understand th...
What would your revision plan be?
bottoming out
Mar 21, 2020
The right implant looks ok.  However the left implant or pocket looks too large, and I am concerned there is extra fluid in the left pocket.  I think the implants you received were too large for your frame, at least initially, which caused a weakness in your left inframammary fold.  So I would include a reduction in your implant size if a revision is to be done.  A capsule repair might do the trick, but it may not, and I would be prepared to either do a mesh or allograft suspension if the c...

Feb 7, 2020
Yes you could be a good candidate for liposuction of the knees, which is not an uncommon request.  Best of luck!
Is a tummy tuck necessary?
Lipo and BBL
Feb 6, 2020
Based on your photo and brief question, I believe decent results can be obtained by performing a BBL and lipo without a tummy tuck.  That being said, based on your skin and stretch marks, I would estimate small chance of having to do a tummy tuck eventually based on the inability of your skin to shrink enough.  It really is based on your expectations, which affects the answer I would give you.

Removing implants
Feb 6, 2020
I think beauty is in the eye of the beholder.  While we as plastic surgeons specialize in beauty, it is much more about what your own personal goals are, are they realistic, and can they be achieved successfully.  Each treatment is personalized and customized for every individual that I meet and at least one full consultation is needed to have that dialogue.  To answer your question specifically simply based on photos and your concerns will be limited, but I do believe you can achieve the...

Sutures for facelift
Feb 6, 2020
Thank you for your question.  I believe what is more important than the choice of suture used is the trust you have in your surgeon, which should have come through a thorough consultation and robust discussion of what to expect.  I personally may change the choice of suture based on the situation since I treat every single patient with a customized and personalized approach.  What may be a good solution for one person may not be the best for another.  As far as tragal incisions, I be...
Will an exposed implant happen a second time if I increase from 1000cc to 1500cc?
implant exposure
Jan 16, 2020
Based on your brief description and photos, the likelihood of complications, including implant exposure is probably near 100%.  Furthermore, going to 1500 cc, from my understanding, is off-label, meaning beyond the manufacturer's recommendations regarding implant size and fill.  I would strongly caution against going any larger, if you are relatively stable at the moment.

nose and lip lift
Jan 3, 2020
I believe you have two distinct issues that can be discussed and addressed.  There are many reasons why you perceive "increased nostril visibility", from the nasal tip height, to the increased columellar droop, to the thick alar rims.  These fine aspects to your nasal tip should be discussed in detail because there are many aspects that are normal and pleasing, so we would want to preserve these.  As far as your upper lip height, a lip lift which involves excising the upper portion of the li...
How common is tachycardia during rhinoplasty and can it be managed?
high heart rate
Dec 16, 2019
I understand your concern about tachycardia after elective surgery.  The most common cause, which is still somewhat uncommon, is related to local anesthetic (particularly epinephrine) or other medications given during your surgery. This temporary phenomenon is usually transient and self limited, and treatment is just observation and fluids, and of course, stopping any further injection of anesthetic.  More rare or uncommon cases result from undiagnosed heart problems or significant family h...
