Following medical school, Dr. Breslow spent 6 years training at the Hospital of the University of Pennsylvania’s nationally renowned Integrated Plastic Surgery Residency Program. There he received extensive training in both cosmetic and reconstructive surgery from some of the nation’s top practitioners. After leaving Penn, he returned to NYU Medical Center to spend one year as the Microvascular Reconstructive Fellow at NYU’s prestigious Institute of Reconstructive Plastic Surgery.
Dr. Breslow is Board-Certified by the American Board of Plastic Surgery, is a member of the American Society of Plastic Surgeons, is a Fellow of the American College of Surgeons, and is licensed to practice plastic & reconstructive surgery in both New Jersey and New York.

If you are concerned about a scar in your armpit and your areolas are small, then I would strongly recommend having the incision at the base of the breast.  It is the most common incision with the least complications, and the scars are well-hidden.

Breast augmentation is not a permanent cure for small breasts as your breast will keep changing over time.  It is not necessary that you wait on having children before having a breast augmentation, so long as you understand that your breasts will likely change size and shape, and more surgery may need to be performed at some point in the future after you have your children.

We recommend that our patients start massaging their breasts 2 days after breast augmentation, 3 times a day for about 5 -10 minutes.  We recommend continuing this for 6 weeks.  You should do it in front of a mirror to gauge the effect that it is having.

Great question.  Basically, if your breasts are too small they need to be augmented.  If they're too low, they need to be lifted.  If they're small and low, then you need both. Trying to lift a breast with an implant is tricky, and only works to a limited degree.  Going too large to achieve this has the opposite effect as the weight of the implant will cause the breasts to drop.

Unfortunately, your breast-breast distance is set and cannot be changed or moved closer (trying to do so with implants will create symmastia, which is a big problem).  Properly placed implants, however, can increase cleavage, which will give the appearance of the breasts being closer together.

We recommend no exercising for a week after a breast augmentation, then you can resume exercising as tolerated so long as it does not involve your chest muscles.  Avoid exercises that use your chest muscles for 8 weeks, then gradually ease back into it.

Breast implant sizing is very important before a procedure.  We have our patients try on a range of sizers, starting from small to large, and then show on Vectra 3D imaging what you would look like after with the selected sizes.  We cannot say what would be an A, B, C, or D because it all depends upon the amount of breast tissue you have to start.  The difference between a 350cc and 400cc within the same profile has more to do with the width than the projection.

A capsular contracture is where the breast implant capsule contracts around the implant and distorts the breast.  The fact that your breast is sticking out more may qualify, but something only needs to be done if it is significantly affecting the appearance or it is painful.  If your implants are silicone, they probably should be exchanged at this point, as it has been 10 years.

I tell my patients not to look at their breasts for size, shape, or symmetry for 6 weeks after surgery.  The breasts will go through many changes, and it really takes 6-12 weeks before they start to take their final shape and look.

Bras do not cause sagging breasts.  Breasts sag as a consequence of the aging process and gravity.  When you age, your skin loses its elasticity and decreases the ability to hold the breasts up.  Bra's may slow the rate of sagging to a minimal extent, but the only way to correct it is with a breast lift.

A rhinoplasty would correct all of these issues.  The hump would be reduced, the bulbous tip would be refined and lifted somewhat to decrease the hooked appearance, and the nose would be straightened in the process.  Fees for this vary by region and by surgeon.

It sounds like you had a seroma that wasn't going away so a drain needed to be placed.  I would not be concerned so much right now with a lump there as it may just be healing tissue.  You should massage the area a few times a day.  Your incision looks like it needs to be debrided/cleaned up a bit.  I recommend calling your surgeon and going back for a follow up.

If you are happy with the amount of breast volume that you have, then a lift alone would be recommended.  From your photo, it looks like you still have sufficient breast volume to give you a very nice shape.   An augmentation would add some more volume, but it would also contribute to the breasts sagging again over time.

From your photos, the area looks a little red and swollen.  It is possible that that is the beginning of a cellulitis, or skin infection.  I would call your surgeon and have him/her take a look.  You may need to be put on a course of oral antibiotics.

The short answer is yes - you are too young for a brow lift.  Unfortunately, the aging process is chronic and progressive.  You are now about to enter your 30's, which is worse than your 20's.  Your 40's will be worse than your 30's.  And so on...  At some point you will need a brow to address the issue.  For now, keeping up with maintenance botox is your best option.  Proper skin care is also suggested to slow down the aging process.
