Can you remove dorsal hump and widen your nose at the same time?
Dorsal hump removal and widening of the nasal bridge
Feb 2, 2019
Feb 2, 2019
Removal of a dorsal hump composed of nasal bone, cartilaginous septum, and bilateral upper lateral cartilages will result in a widened appearance of the nasal bridge if spreader grafts are placed and no osteotomies are performed.
To fat graft or not with facelift?
Facial Fat Grafting with Face / Neck Lifting
Feb 2, 2019
Feb 2, 2019
Facial Fat Grafting is an ideal solution for volume loss in many key areas of facial aging, including the temporal hollows, periocular region, cheeks, and mandible. Why? Your fat has a zero chance of rejection or tissue reaction. Your fat will revascularize and approximately 50% will last for many years, far longer than HA fillers. Placement is the key. I place all fat grafts deep. Placement in tear troughs and thin skin areas are risky for visibility and/or contour issues. Lastly, normal...
How can I reduce internal scar tissue after a rhinoplasty surgery?
Scar tissue formation following revision rhinoplasty
Dec 15, 2018
Dec 15, 2018
Kenalog is my first line treatment for thick scar tissue combined with massage. Since you have had many procedures, you will have a greater amount of scar tissue than a primary rhinoplasty patient. Several other surgeons have mentioned accutane as a treatment for your thick oily skin. Accutane is an oral (systemic) treatment with systemic side effects that you may not be willing to accept if you do not have severe facial acne outbreaks. Consider treating your nose with a topical retinoid li...
Will my nose bend back if I use my own rib cartilage?
Postoperative Rib Cartilage Warping or Bending
Dec 15, 2018
Dec 15, 2018
Rib cartilage is very strong, abundant in supply, and has several advantages when septal cartilage is insufficient for nasal structural support. Rib cartilage bends (warps) when carved. Your rhinoplasty surgeon can use this property to their advantage to ensure your nose has the best chance of remaining straight over the long term. I use opposing extended spreader grafts for this purpose but there are many methods to achieve the desired result. Discuss your concern with your surgeon and he...
What would you recommend for under eye area bags at 37 years old?
Under eye bag treatment
Oct 21, 2018
Oct 21, 2018
Start conservative. Filler can easily be reversed but usually provides an excellent cosmetic outcome. Botox will help with lateral (crows feet) wrinkles.
I have folds on oustide of eyes when I smile. Is there anything that can be done?
Lower eyelid wrinkles
Oct 21, 2018
Oct 21, 2018
Botox applied to the lower eyelid and lateral orbicularis muscle folds should improve these rhytids. Give it another try, preferably from a surgeon who understands eyelid anatomy. If that doesnt work, a skin pinch blepharoplasty and chemical peel is your next best option.
I would like to have a rhinoplasty/septoplasty done. What are the suggested ways of achieving my g
Tip Rhinoplasty
Oct 21, 2018
Oct 21, 2018
Your nasal upper two thirds require very minor refinement as the bones and septum appear straight and you have a very slight dorsal hump, this could easily be performed via intra nasal incisions only. The lower 1/3 (tip) is what I think you would like to change the most. My recommendation would be an open rhinoplasty approach (columella incision) as you have large lower lateral cartilages that will require significant modification. Slight reduction in the width of your nose (base reduction)...
31 year old looking for surgeon in Austin, TX area experienced in working with hereditary hooding
Upper Blepharoplasty for hereditary hooding
Sep 7, 2018
Sep 7, 2018
I also recommend a skin only, conservative upper blepharoplasty. You have great brow position. However, your medial canthus resembles an epicanthal fold seen in Asian patients. You will need to consider keeping or permanently changing this region to a more Caucasian appearance as this is extremely difficult to replace. This procedure can easily be performed under local or local + conscious sedation in the office.

Upper eyelid scar revision
Aug 9, 2018
Aug 9, 2018
You will always have a scar, but the depressed and widened appearance can be minimized with scar revision so that it heals flat, thin, and white. Sun exposure avoidance is highly recommended following the procedure to minimize the chance of scar pigmentation.
Can my crooked nose be fixed?
Straightening a crooked nose
Jul 24, 2018
Jul 24, 2018
Straightening a crooked nose usually requires modification of the nasal bones as well as the nasal cartilage support structure. Straightening a deviated septum is often part of the operation and is the preferred source for spreader grafts, etc., needed to complete the procedure. The goal of surgery is to improve your nasal airway and meet your cosmetic goals. Thank you for your question.

Embedded Earlobe foreign body
Jul 22, 2018
Jul 22, 2018
Removal of an earlobe foreign body is a relatively straight forward procedure. The lobe is first numbed with local anesthetic via two or three injections with a very fine needle. Next, an incision is made that provides the best access to the object and the best cosmetic location. Long term foreign bodies will have a capsule around it that is usually removed with the object. Deep sutures are placed followed by skin sutures. Downtime will be minimal and you can expect a very inconspicuous scar...

Any postoperative concern for infection
Jul 8, 2018
Jul 8, 2018
Any time you have a concern regarding a potential post op infection means a trip in to see your surgeon or at least communication with him or her. This could be simple milia formation or a stitch abscess, an in person examination is the best way to diagnose and treat. Send your surgeons office these pictures today and/or call, they may start you on antibiotics prior to your next follow up. Thank you for your question.
What is the best way to remove a raised facial mole?
Best way to remove a raised facial mole?
Jul 6, 2018
Jul 6, 2018
Raised facial moles usually require full thickness excision and primary closure with sutures for the best cosmetic outcome. You will have a scar with any method of removal, however, surgical incisions that fall within relaxed skin tension lines and meticulous closure techniques generally heal so well that light foundation with sunscreen is all that is needed to camouflage the fine scar line. I would not recommend a shave because this will result in a round scar the diameter of the base of ...
Most effective treatment to address skin laxity on neck and jowling? Face lift or other?
Skin laxity and neck rejuvenation
Jun 30, 2018
Jun 30, 2018
Based on your limited photographs, you have skin laxity with minimal subcutaneous fat as well as platysmal banding and a good hyoid bone position. A conservative SMAS lower face lift and platysma plication would give you the best rejuvenation results and address your main issue, excess skin.If you are only interested in non surgical options, laser or radiofrequency options would be recommended.Thank you for you question.
Is telogen effluvium possible following a septorhinoplasty (3 hours, general anesthesia)?
Post surgical telogen effluvium
Jun 27, 2018
Jun 27, 2018
Any significant stress (physical and/or psychological) on the body can cause telogen effluvium in susceptible individuals. The only FDA approved hair loss medication, for use in women of child bearing age, is Rogaine (Minoxidil). I recommend using the OTC 5% version. It will take several months of use to see a significant improvement.
