Your pictures are clear (thank you for that), and so it's easy to see that you have a significant breast ptosis with your nipple falling below the breast fold. As such, a breast lift is absolutely indicated, either with or without an augmentation, depending on your desires and expectations.

Fat augmentation alone should not be performed on you, as it will just make your breasts look heavier and more ptotic (droopier). A breast lift is absolutely indicated, and could be combined with a breast augmentation, fat transfer, both, or neither, depending on your desires.

I'm not so sure that intra-abdominal adhesions after multiple cesarean sections correlates with capsular contracture after breast augmentation. It's best to see a surgeon with vast experience in breast augmentation and also in breast revision surgery, and ask what their capsular contracture rate requiring surgical correction is.

Based on your pictures, I do believe that you would need a lift as well as a breast augmentation with implants. If you try to skip the lift, you most likely won't be happy with the results. See a board certified plastic surgeon in your area for a consultation and to further discuss your options.

Silicone gel implants typically do not ripple as much as saline implants, and implants with a higher fill are going to be more firm than those with a lesser fill. This causes them to ripple less. That said, if you are very thin and get large implants placed, you are more likely to have rippling.

You do have asymmetry in your breasts, and you would benefit from an augmentation, but you really should consider a small lift at the same time. Based on the pictures, you most likely do not have a significant tuberous breast deformity.

The 400cc HP implant has approx 20% more projection than a 450cc MPP, and the MPP has a larger width, so they will look significantly different. HP 5.0cm, MPP 4.2cm

I agree that the position of your nipples is too high for your breasts, and you should discuss this with your surgeon. Changing implant size and shape should help, but you may also need to have correction of the pocket, due to implant bottoming out.

While this may look scary to you, this happens with surgery sometimes.  I would recommend that you follow the advice of your surgeon.  If it heals poorly, you can have a discussion with him about revision in the future.
Would 300 or 325 cc be the best size for me?

At 96 lbs, you're quite petite.  Either of the two implant sizes that you have chosen are going to make a wonderful change for you, but if you want to be a C, you're better off with the smaller size.  As you know, bras fit differently, and in some bras, your size will be a D.
