Typically you would "take care" of any septal perforation and nasal valve collapse at the same time. Although the parts of surgery are broken down into different components (i.e. septal perforation repair, nasal valve repair, etc) for medical and billing purposes, there are few instances where repairing septal perforation would not also include addressing the nasal valves.

After multiple nasal fractures and surgeries, you need to consider your goals and create realistic expectations.
You should expect a great looking nose that is relatively straight. But to get, there you likely need an extensive revision of your entire nasal framework (cartilage and bone). This will likely involve cartilage grafting but I will demur on the specifics as any surgeon, myself included, would need to evaluate you in person to get a true sense of what might be needed....

Cephalic trim via a non delivery technique can be a great, less invasive technique to help refine the nasal tip. From your picture it appears that your tip cartilages sit in different positions (the right looks higher than the left). The asymmetry or fullness you are concerned about comesfrom this. Cephalic trim alone is unlikely to solve this problem. I am not sure a non delivery Cephalic trim approach to your nasal tip is going to get you the result you want.

My office and I recommend patients abstain from strenuous physical activity for 2 weeks after surgery. This helps minimize swelling of the nose. Keep in mind your nose will have some swelling for up to a year after surgery. Any thing you can do to minimize swelling early will help you later.
