Hi globemom, thanks for your question. Capsular contracture is a tough problem that we Plastic Surgeons have been working on for years. No one knows exactly what causes it, or how to prevent it completely.

Hi KG2302, thanks for your question. Just adding a larger implant is probably not the way to go to fix your wide-set breasts and improve cleavage. This has more to do with the adjusting the position of the pocket for the implant. This may be able to be changed, but a physical exam would be necessary to tell for sure.
