From your photos I cannot tell if you have an open roof deformity. Your skin is moderately thick in that region and you may still have some swelling. Typically it is easier to palpate an open roof deformity in someone with moderate skin thickness than it is to see it. Also an open roof deformity does not necessarily widen your nose, so again an exam is necessary. Please discuss with your surgeon or seek a second opinion.

This really depends on the complexity of your rhinoplasty, your questions, and the surgeon's questions. For most cases, this takes about 15-30 min, but if more time is needed then that is ok. You should feel like you have all of your questions and concerns addressed by the time of the visit. Sometimes a second visit is necessary to further discuss goals and expectations of surgery.
Why can't I breathe after getting a rhinoplasty 10 months ago?

You mentioned rhinoplasty, but did you also undergo a septoplasty or functional aspect to improve breathing? If not then the surgery should not affect your ability to breathe and the difficulty could be caused by something else, such as congestions, masses, prior deviation of the septum, concha bullosa, etc. You need to be examined to find out. Would I benefit from a donut breast lift?

100% disagree. Your breast ptosis is pretty significant, and it needs a more significant lift than a donut mastopexy, or you will be left dissatisfied. Additionally, outside of very small breast lifts, a donut hole mastopexy can negatively impact the shape of your breasts, whereas circumvertical and wise pattern mastopexies typically do not impact the shape in a bad way.

Consider a second opinion with another surgeon you trust, has good results, and is board certified in plastic surgery.

1 in 19,000 is extremely unlikely! I would not worry about it. Additionally, breast lift surgery does not require paralyzing drugs, and therefore if you were in any way light under anesthesia your body would move and the anesthesia team would put you deeper under anesthesia to make sure you're not awake or uncomfortable. A lot of surgeons use local anesthesia when able to because it makes the procedure more affordable for the patient. In my practice, I prefer to do most of these under general anesthesia for comfort also, unless the breast lift alone is very small, only involves the areola, and perhaps a small area around that. Otherwise the patient may be uncomfortable. If your surgeon is hesitant to use general anesthesia, I would recommend meeting with another surgeon for a second opinion. Good luck!

Those are all great questions. The good news is that you do not have to do anything. It is not harmful, and bottoming out to some extent after surgery is always expected. Some people bottom out more, and some of it has to do with tissue quality. If you're thinking of getting pregnant, I would not recommend surgery before pregnancy, as pregnancy is highly likely to impact your outcome. I agree this is not an ideal outcome, but it can only be compared to your preoperative photos to have a better idea. Otherwise we're comparing them in a vacuum. Your breast tissue has limited volume, so no matter how great the lift is, it may still look a bit deflated. If that is what bothers you, you may benefit from an implant. Unfortunately, these two issues (breasts and hernia) are unrelated and I would not suggest having a breast lift if you're hoping that it will help with your hernia. If your hernia is causing trouble I would recommend hernia repair. If the breast ptosis or size bother you, then I would recommend first doing a breast lift or breast reduction.

You have small breasts. I do not see a tuberous breast. They fit your body and body type. If you wanted them to be larger you could always get breast implants, but I would not do that simply because someone told you they're not "attractive." Only do it if you are bothered by their size. It's impossible to know without knowing your breast width, tissue thickness, stretch, etc. those are all important measurements that help determine the best range for a good fit that achieves your outcomes. The range you listed may work, but ultimately it comes down to checking all the measurements and making sure that it could work for you. Trust your surgeon to do a good job with this. If your surgeon rushes through your visit and you don't trust his or her decision, see someone else. At the end of the day it's your body and this is elective surgery, so you should be able to trust the surgeon you choose. The implants you listed are pretty big for your size, but depending on chest wall width etc they may still look good and achieve the desired result you want. Your breasts look good 2 years out. You have to be more specific about what you are particularly bothered by and we can be of more help, but overall they have some natural ptosis, look even, don't have a visible "snoopy" deformity, or any of the other known possibilities that I can appreciate from the photos you posted. An in person examination is much better at helping to determine what is bothering you and whether something surgically should be considered. Talk to your surgeon to see.

Whenever a patient has an implant removed, there is always a small space that remains, often permanently. These spaces often have a little fluid in them called a seroma and it's no big deal. Unless you have signs of an infection, it is safe to proceed with revision breast surgery. Good luck!

The difference between 385 and 415 cc on your body is smaller than you think, and the 385 cc will likely look good on your frame. Hard to know if it will be a "D" cup as that varies by manufacturer, but it should be something in that range. I think you'll be very happy. Good luck!
