What kind of nose job is best?
"Best" nose job
Feb 20, 2017
At the risk of sounding cliche, there really is no "best" rhinoplasty because that varies from person to person- it is a combination between what your goal is, and a nose that looks natural and fits your face. That being said, I can see from those photos what your concerns are (thanks- those help tremendously). I agree about the tip- if that was left alone, and only the bridge was changed, the tip would look even more pronounced. I would recommend reducing the bridge to...
Is it possible to make the nasal tip less pointy and protruding with little to no alteration to
Making nasal tip less pointy
Jan 28, 2017
Yes, it is possible to make the tip of the nose less pointed without significant change to the front view. HOWEVER, it all depends how it looks now, and how much you want to reduce it. Sometimes this is a very straight-forward procedure, but can be tricky in certain circumstances, again depending on the anatomy. I know that sounds like a cop-out answer, but it is the truth. When going for a consult, be sure to request digital imaging to simulate results- this...
Between all of the injectables: Botox, Dysport and Xeomin, do any of them cause less bruising?
Less bruising with Botox, Dysport or Xeomin?
Jan 28, 2017
No. The risk of bruising is the same with Botox, Dysport, or Xeomin. For people prone to bruising, I find that applying ice before and after the injections is very helpful. Also make sure you are not taking products such as ibuprofen, aspirin, or Vitamin E, as these can increase risk of bruising.
Wrinkles and microdermabrasion? I would like to smooth the 11's on my forehead.
Quickest easiest fix for forehead lines
Jan 28, 2017
The easiest way to improve those "11's" on the forehead (aka frown lines or furrows) with the least risk and least interruption to your daily routine is to get Botox or Dysport injections. Those lines are typically caused by muscle use, and Botox or Dysport will weaken the muscle (not permanently). You're in and out of the office typically in 15 minutes, and can return to your day immediately. Results last generally 3-4 months. If lines are very deep, they may also...
Can an ophthalmologist with no training and no board certification in surgery perform facelift?
Can ophthalmologists perform facelifts?
Jan 11, 2015
Technically, any licensed physician (including providers such as family practitioners or ob/gyn physicians) can legally perform a facelift, so long as they do not misrepresent themselves to the patient. But SHOULD they be doing it? My opinion is absolutely not unless they have specific training in that. Their board certification should reflect it. This is referred to as "performing within the scope of practice/expertise of a physician's training." Some...
How do I verify that my Mid-Face Lift will NOT be a Lifestyle Lift?
Midface Lift vs Lifestyle Lift
Jan 11, 2015
The most reliable way to avoid having a "Lifestyle Lift" is to contact that company (website or phone) and get a list of their participating surgeons, so you know who's who. Just searching for a "board certified anyone" (whether they be plastic surgeon, facial plastic surgeon, cosmetic surgeon, oculoplastic surgeon or dermatologic surgeon) does NOT guarantee they arent providers for Lifestyle Lift. By definition, a Lifestyle Lift is not the same as a midface lift. Dr....
Are cartilage grafts on the bridge to widen the nose safe?
Cartilage grafting to widen the nose (spreader grafts)
Aug 3, 2014
This is a very safe, reliable, and time-tested way to correct the problem of a overly narrow nose, or collapse of the midportion of the nose, whether it be from prior surgery, trauma, or just a natural occurrence.
Can men have concave profiles in rhinoplasty? Or do all men have to have a convex profile?
Difference between masculine and feminine nose
Aug 3, 2014
Yes, men can have a concave profile to their nose. In general, a straight or convex profile is associated with a male face, while a concave profile is associated with female characteristics. This is not a rule, and exceptions for both genders abound (Barbara Streisand and a younger Kevin Bacon are good examples). The shape just changes the overall nature of the face. The best way to see this is with computer imaging (or just try using Photoshop at home, and play with...

Revision Facelift After Bad Results
Jun 28, 2014
Based on those photos, the jawline and the neck skin/muscle laxity are what stand out to me. Having pre-op photos would help to assess the degree of change, but if I were basing my decision based solely on those photos, I would be recommending a traditional rhytidectomy (lower face/necklift) with tightening of the platysma below the chin to achieve a smooth contour. Nothing fancy or trendy- it is a time tested reliable method. If you haven't yet, I would advise bringing...
2 weeks post op of Neck & Cheek lift, my left side of is swollen and dented. Is this normal? (p
Irregularities 2 weeks after facelift surgery
Jun 28, 2014
At 2 weeks after a facelift or neck lift, it would be too soon to panic, based on that photo. Most irregularities will settle as the swelling resolves, and the skin/soft tissue relaxes after the pull. I would advocate gentle manual daily massage at that dimpled area, but definitely consult with your surgeon first, as he knows exactly what was done surgically.
Why would doctor do a short scar facelift with incision in front of tragus as opposed to behind
Facelift incision in front of ear (pre-tragal) or behind the tragus (post-tragal)?
Jun 28, 2014
This is decision is usually by surgeon's preference. Both heal very well, and if you don't like the outcome of an incision made in front of the ear, it can easily be converted under just local anesthesia. In the male face, the preference is to make a pre-tragal incision so as not to pull the beard line back onto the ear. With the female face, either incision can be made with good results, so long as the closer is meticulous.
How long does the numbness after a facelift stays?
Numbness after Facelift
Jun 28, 2014
Numbness is to be expected after a facelift, or neck lift (aka rhytidectomy). Depending on the exact surgical technique, it usually involves the skin in front, behind, and below the ear as well as the area below the chin. Most sensation returns over several weeks, but there can be smaller patches that will take another 12-18 months. I always inform patients of this before surgery so there are no surprises. What should NOT be numb is the actual ear itself. This...
I have asymmetry after face/neck lift. What are the my chances of it resolving itself?
fullness below jaw after facelift
Jun 28, 2014
This early after a facelift or neck lift (aka rhytidectomy), there can be some irregularities in contour, often due to the post-op dressing and retained swelling or healing tissue. They usually self resolve over time, and with the application of warmth and daily manual massage. Sometimes they require steroid injections, and less often, revision surgery. But this soon after surgery, a conservative approach is the prudent way to go based on that photo. I'm sure you have...
Is it normal to see tip cartilage grafts through nostrils post-op?
Nasal tip cartilage graft for rhinoplasty
Jun 22, 2014
Cartilage used for tip grafts in rhinoplasty should NOT be visible at any point after surgery. If they are exposed (i.e. not covered by the skin on the outside of the nose, or the mucosal lining inside the nose), you need to contact your surgeon to let them know this. These may not be pieces of cartilage either- it could be a bolster dressing or silicone splints, which is more likely, and not an emergency at all. But either way, contact your surgeon to let them know.
Will exercising affect my Rhinoplastery results?
Exercise after rhinoplasty
Jun 22, 2014
Five months after a rhinoplasty, exercising should have no effect on the long-term outcome of your rhinoplasty (unless you accidentally hit the nose with something like a weight bar). You will likely have some post-workout swelling because of the increased heart rate and blood flow to the face, but this is temporary. I usually advise my patients to resume light exercise even at the first 1-2 weeks after surgery, then back to their full routine at one month.
