Why have 3 plastic surgeons dissuaded me from getting Rhinoplasty? Is my case that difficult?
Rhinoplasty consultation
Nov 5, 2020
Thanks for your question. What you are asking for is very common and seems doable for you with surgery. In fact, these are typically the main goals I'm hoping to achieve in my own patients. While I cannot speak for the surgeons with whom you met, I would think rhinoplasty would be reasonable to consider. The most important factor towards success is appropriate expectations. Digital simulations are helpful to ensure you and your surgeon are on the same aesthetic page. Best of luck! - Dr. C.
Lip reduction effect on nasolabial angle?
Lip reduction effects on nasolabial angle
Aug 21, 2020
Hi, and thanks for your question! Without photos it's difficult to give you personalized advice, but in general a lip reduction has only minor effects on the nasolabial angle. You are correct that removing lip mucosa and subsequent suturing will roll the lip inward but this usually doesn't have a significant impact near the nose. That said, there are several components of that angle, including the base of the nose, the bony structure beneath, and the lip itself; all of which can be modified...
I have a flat nose, but when I smile the tip curves and drops down. Can I change it naturally
Droopy nasal tip when smiling
Mar 25, 2020
Nope. They don't work. The changes you see when smiling are a combination of poor tip support and your smiling muscles pulling up your cheeks. Also, there is a small muscle called the depressor septi muscle that can sometimes pull the nose down a bit. This can be relaxed with Botox or divided during surgical rhinoplasty. An in-person consultation with a board-certified rhinoplasty specialist would be very helpful as a next step. Best of luck!
Is there enough septal cartilage for a rhinoplasty revision?
Cartilage source for revision rhinoplasty
Mar 25, 2020
Hi, and thanks for your question. It's a really great question and something with which nearly every revision rhinoplasty patient is concerned. Generally speaking, you can get a good idea about how much septal cartilage remains during the in-person physical exam. I shine light through the septum, and use a small Q-tip to feel along it for resistance. If it feels floppy then we know there is minimal cartilage there. Unfortunately, imaging studies like CT scan,...
I'm looking for a treatment for extreme dark circles, any suggestions?
Dark eye circles
Mar 23, 2020
Thanks for your question. Unfortunately, the dark pigment you have is especially difficult to treat. You've already had the most common procedures without much improvement. I doubt RF devices would make much of a difference. That said, it seems some of the darkening is due to shadow, which could certainly be improved with filler or fat transfer. A formal surgical lower blepharoplasty would give you the best result. I recommend you see a board-certified facial plastic surgeon...
What procedure(s) are available for me to fix my nose and chin?
Nose and Chin
Mar 20, 2020
Thanks for your question and photo! Regarding your nose, filler or "liquid" rhinoplasty may be an option to add volume above the bump and create a straighter profile. However, it will create a larger nose that may not be balanced with the rest of your face. Ultimately, I suspect the nicest shape would be achieved with surgery which would allow the bump and tip to be brought inward for a better relationship with the top of your nose. Filler obviously can't do that.

As for your...

What can I do to reduce the size of my chin?
Chin reduction
Mar 19, 2020
Hi, and thanks for your question! First off, I think overall you have a beautiful profile and nothing is significantly out of balance. Strong chins are a hallmark of classic facial harmony. That said, if you preferred a less pointed chin then chin reduction would be possible. It's performed through either an oral incision, or a small incision under the chin. The excess bone is then carefully shaved down to create the desired shape. Healing is generally straightforward with minimal...
Are these rhinoplasty expectations realistic? Follow up to an earlier question
Rhinoplasty expectations
Mar 18, 2020
Thanks for your question. I would say that based on this photo your expectations are very reasonable. Obviously, additional angles would be helpful to give you more specific advice. Make sure you see a board-certified rhinoplasty specialist who also performs functional nasal surgery to get the best result. Good luck! - Dr. C
Are silicone injections, any other type of injection, or ear grafts the best option for lower
Retracted alar rims
Mar 16, 2020
Thanks for your question. Without photos it is impossible to give you specific advice, but generally speaking, injections alone usually don't have enough strength to lower alar rims to any significant degree.  Depending on the amount of retraction, the best option includes cartilage grafts (from either septum or ear), and in some cases, composite grafts from the ear (including skin and cartilage). These are permanent options that look natural when done well. Cartilage grafts are your own t...
What's the least invasive and effective procedure to get rid of under chin patch?
Under chin fullness
Mar 16, 2020
Thanks for your question and for including your photo! The least invasive option is Kybella, but it requires several sessions to get a good result, is less predictable, and each session has about a week of swelling and discomfort.  I would recommend micro-liposuction, which is performed through tiny incisions that nearly disappear.  You could consider combining that with something to tighten the skin like Vivace microneedling, FaceTite, or Renuvion (helium plasma device).  As always, I rec...
Should I replace a chin implant from 1992 that has begun to recede, or do fat grafting?
Replacing a chin implant
Sep 17, 2019
Thanks for your question. There are many factors that go into this decision. An old chin implant can certainly be replaced but the best course of action would depend on what's found during a physical exam, and perhaps imaging studies to assess the quality of the underlying bone.  As we age the mandible slightly recedes with time. Combining this with early jowling that develops around the sides of the chin creates the illusion of chin regression.  That said, replacing the implant with a la...
Can I get Botox/Dysport during my period/menstrual cycle?
Timing of Botox
Sep 17, 2019
Should be no problem whatsoever. I would not expect any issues directly related to the Botox. Thanks for the question.
How soon after dermal fillers in my chin can I safely have a chin implant procedure performed?
Chin reimplantation
Sep 6, 2019
Having the filler in place makes it difficult to choose the correct size implant for your permanent result. I would recommend either having the fillers dissolved or waiting until they go away.  Infection risk is not necessarily related to previous infection but is always a possibility with this procedure. Best of luck.
What are the options for correcting bad surgery?
Airway obstruction after nasal surgery
Sep 3, 2019
Sorry to hear you are uncomfortable with your new nasal airway. At three weeks there is likely still significant swelling causing added obstruction within your nose.  It is best to visit a board certified septorhinoplasty surgeon to have a complete physical exam performed to see if anything further needs to be done. Best of luck!
Is bone resorption stops once you remove the chin implant?
Bone resorption
Jun 15, 2019
Yes, the resorption stops after the implant is removed. This is a rare complication usually seen when implants are not secured in place. However, bone grafting can be done to correct the defect if necessary and a new implant can sometimes be placed.
